生物反馈对功能性便秘患者心理状况生活质量及自主神经功能的影响

摘　　要：目的探讨生物反馈对功能性便秘(functional constipation,FC)患者精神心理状况、生活质量和自主神经(autonomic nervous system,ANS)功能的影响。方法应用Zung’s焦虑自评量表、Zung’s抑郁自评量表、汉化版简明健康调查量表(SF-36)、HANS-1000型自主神经生物反馈仪,分别评定21例FC患者生物反馈治疗前后精神心理状况、生活质量和自主神经功能。结果生物反馈治疗后,(1)FC患者焦虑、抑郁状况较治疗前显著改善(P<0.01);(2)生活质量的总体健康、生理功能、情感职能、躯体疼痛、活力5个维度分值均有明显提高(P<0.05),且总体健康和情感职能改善尤为显著(P<0.01),提示FC患者生活质量显著改善;(3)FC患者自主神经功能并无显著改变(P>0.05),提示本研究尚不能证明生物反馈可以改善FC患者自主神经功能。结论生物反馈可以改善FC患者心理状况、提高生活质量;生物反馈改善FC是否是通过自主神经起作用,还有待进一步研究。Objective To explore the influence of biofeedback therapy on the psyche, quality of life and autonomic nervous function of patients with functional constipation. Methods The psychological status, quality of life and autonomic nervous function of 21 cases with functional constipation （FC） were evaluated with Zung Self-Rating Anxiety Scale （SAS）, Zung Self-Rating Depression Scale （SDS）, heart-rate variability （HRV） analysis, a Chinese version of the MOS 36-item short-form health survey（SF-36） and HANS- 1000 autonomic nervous biofeedback apparatus respectively before and after biofeedback. Results After biofeedback therapy, （1） the scores of SAS and SDS in FC patients were markedly improved （P〈0.01）; （2） the scores of GH, PF, RE, BP, VT were increased significantly（P〈0.05）; especially the scores of GH and RE（P〈0.01 ）, which indicated the quality of life in FC patients was significantly improved; （3） there were no significant difference in autonomic nervous activity （P〉0.05）, which showed this study could not confinn that biofeedback therapy can improve the autonomic nervous function. Conclusion （1）Biofeedback therapy can improve the psychological status and quality of life in patients with FC. （2） Further research is needed to make sure whether biofeedback therapy can improve the automatic nervous function in FC patients.
