摘  要:目的:以卵巢肿瘤患者腹水中细胞形态学和组织病理学结果为基础,分析细胞超微结构在诊断中的意义。方法:术前经腹腔穿刺获取48例卵巢肿瘤患者的腹水,分别送电镜室和病理科,经相应过程处理后进行光镜和电镜观察,并将观察结果与术后的组织病理学结果比较。结果:5例良性肿瘤(卵巢卵泡膜细胞瘤2例、成熟性畸胎瘤1例、腺纤维瘤1例、浆液性囊腺瘤1例)腹水光镜和电镜检查结果均为阴性。43例恶性肿瘤(浆液性乳头状腺癌32例、黏液性腺癌5例、上皮型恶性间皮瘤3例、神经内分泌小细胞癌2例、内膜样腺癌1例)腹水的光镜诊断阳性率74.42%;电镜阳性率81.39%。电镜对肿瘤组织来源的诊断符合率83.33%,光镜为56.67%。结论:电镜对腹水肿瘤细胞组织学来源的诊断具有优势,与光镜结合有助于提高卵巢肿瘤临床诊断率。Objective:Based on the results of morphology of cell in ascites and pathology of tissue, we analyse the diagnostic significance of cell ultrastructural on ovarian tumors. Method:In 48 cases, ascites were taken before operation, and they were inspected by microscope and electron microscope separately though treatment rel- evantly. Result :5 cases of Benign tumor were theca cell tumor (2 cases) ,mature teratoma (1 case), adenofibroma ( 1 case), serous cystadenoma tumor ( 1 case). 43 cases of malignant tumor were papillary cystadenocarcinoma (32 cases), mucinous adenocarcinoma (5 cases), epithelial malignant mesothelioma (3 cases), neurocrine small cell carcinoma (2 cases), endometfioid adenocarcinoma (1 case). We refer to the result of pathology after operation. The benign ascites of 5 cases were both negative for microscope and electron microscope. The positive rate of 43 cases malignant ascites were 74.42% in microscope and 81.39% in electron microscope. On origin of tumor histology, the coincidence rate was 83.33% in electron microscope and 56.67% in microscope. Condmion:Electron microscope has superiority for the histological origin of tumor cell in ascites. To combine microscope with electron microscope may improve the diagnosis rate of ascites at ovarian tumors.
