摘 要:目的建立猪SAH模型并研究其脑血流灌注的变化。方法实验猪随机分为SAH组(n=5)和假手术组(n=4)。SAH组在右额颞开颅向鞍上池内注入自体动脉血制成SAH模型,假手术组注入等量生理盐水。4d后行SPECT-CT同机融合扫描并以大脑小脑比(corticocerebellar ratio,CCR)、相对离散度(relative dispersion,RD)半定量描述大脑半球血流量和血流分布离散度。7d后对海马行苏木精-伊红染色,观察神经元形态变化。结果CT证实血液积聚于鞍上池,造模成功。SAH组左右侧CCR(1.768±0.298,1.382±0.192)均较假手术组对应侧(2.131±0.246,1.988±0.346)显著下降(P<0.05);SAH组两侧CCR差异亦有统计学意义(P<0.05)。SAH组右侧RD(0.417±0.015)较假手术组右侧(0.389±0.015)显著增加(P<0.05),SAH组左侧RD(0.406±0.023)与假手术组左侧(0.378±0.030)差异无统计学意义(P>0.05),SAH组两侧RD差异无统计学意义(P>0.05)。苏木精-伊红染色证实SAH组海马神经元形态异常。结论猪SAH模型符合SAH血管痉挛引起的异常脑血流灌注特征,为此类疾病研究提供了理想的大型动物模型。Objectives To establish a pig subarachnoid hemorrhage (SAH) model and observe the changes of cerebral blood perfusion. Methods The pigs were randomly divided into SAH group (n = 5) and sham-operated group (n = 4). SAH model were established via the injection of autologous arterial blood into the suprasellar cistern, while the sham-operated group was injected with normal saline instead. SPECT-CT infusion scans were applied after 4 days, then parameters corticocerebellar ratio (CCR) and relative dispersion (RD) were utilized for quantitation of cerebral hemisphere blood flow and flow dispersion. HE staining was applied to observe the neuronal histological changes of the hippocampus at 7 d. Results CT imaging showed blood accumulated successfully in the suprasellar cistern in SAH group. The left and fight hemisphere CCR (1.768 ± 0.298 and 1.382 ± 0.192) in SAH group were significantly decreased compared with the corresponding side of shamoperated group (2.131 ± 0.246 and 1.988 ± 0.346) (P 〈 0.05). There was significant difference of CCR between the two sides in the SAH group (P 〈 0.05). RD in right side of SAH group (0.417 ± 0.015) increased compared the ipsilateral sham-operated group (0.389 ± 0.015) (P 〈 0.05), however, no significant differences were showed in the left side between SAIl group (0,406 :e 0.023) and sham-operated group (0.378 ± 0.030) (P〉 0.05). No difference of RD appeared between the two sides in the SAH group (P 〉 0.05). HE staining showed that the morphology of hippocampus neurons were abnormal in SAH group. Conclusion The pig SAIl model has a characteristics of abnormal blood flow perfusion which is consistent with the features of vasospasm induced SAIl, thus being an ideal large animal model for research of related diseases.
