摘 要:目的观察鞘内注射(intrathecal,i·t)加巴喷丁和吗啡对切口痛大鼠脊髓氨基酸类神经递质的影响。方法选择鞘内置管成功的雄性SD大鼠40只,随机分为5组,每组8只,分别为假手术组,对照组,加巴喷丁50μg组,吗啡2.5μg组,加巴喷丁50μg加吗啡2.5μg组。按Yaksh法鞘内置管;按Brennan法制作大鼠足底切口疼痛模型;用机械缩爪反射阈值(mechanical withdrawal threshold,MWT)和热缩爪潜伏期(thermal withdrawal latency,TWL)来确定疼痛行为学变化;应用高效液相色谱法(HPLC)测定脊髓氨基酸类神经递质如谷氨酸(glutamate,Glu)、天门冬氨酸(aspartate,Asp)、甘氨酸(glycine,Gly)、γ-氨基丁酸(γ-aminobutyric acid,GABA)含量的变化。结果与假手术组比较,对照组大鼠在术后2h时MWT明显降低,TWL明显缩短(P〈0.05),脊髓Glu、Asp含量明显升高(P〈0.05);与对照组比较,术前i·t加巴喷丁50μg加吗啡2.5μg在术后2h的MWT明显增加,TWL明显延长(P〈0.05),脊髓Glu、Asp含量明显减少(P〈0.05)。结论在大鼠切口痛模型中,鞘内注射加巴喷丁能增强鞘内吗啡的抗伤害作用,其机制可能与脊髓兴奋性氨基酸含量减少有关。Objective To study the effects of intrathecally administering gabapentin and/or morphine on the amino acid neu- rotransmitters in spinal cord of rat incision pain models. Methods Forty male SD rats were randomly divided into five groups (n=8) and were intrathecally inserted catheter. Paw incision models were made 5d later. Gabapentin (50 μg, group Ⅲ ) or morphine(2.5 μg, group IV ) or both (group V ) were intrathecally injected 30 min before incision. Another two groups were regarded as shame (group I ) or control (group Ⅱ ). Pain behavior was determined 2 h later using mechanical withdrawal threshold (MWT) and thermal withdrawal latency (TWL). The content of four amino acids including glutamate, aspartate, glycine and γ-aminobutyric acid in rat spinal cord were detected. Results MWT and TWL decreased and the content of glutamate and aspartate in spinal cord increased significantly 2 h after incision(P〈0.05 )in groups Ⅱ, Ⅲ and IV(without statistically differences among three groups), but not in group V, compared to them in group I. There were no differences in the content of glycine and γ-aminobutyric acid in rat spinal cord 2 h after incision among four groups. Conclusion Intrathecal gabapentin significantly enhances the antinociceptive effect of intrathecal morphine in rat incision pain model, which may owe to the decreased level of excitatory amino acids in spinal cord.
