摘  要:目的探讨女性抑郁症患者额叶灰质密度与执行功能及相互间关系。方法30例女性重性抑郁症患者(首发14例,复发16例)及16名女性正常对照,均行三维核磁共振(3D)扫描和认知功能测试。结果多重两两组间比较显示,首发组较正常组及复发组额叶灰质密度的差异均无统计学意义(P>0.05),但复发组左额上(P=0.03)、右额上(P=0.02)及右额中(P=0.02)的灰质密度较正常组降低。复发组威斯康辛卡片分类测试(WCST)完成分类数、错误应答数和持续性错误数、数字广度及数字符号成绩均差于正常组(F为3.81~11.18,P均小于0.05),其右额下的灰质密度与错误应答数、持续性错误数负相关(r=-0.51,P<0.05;r=-0.53,P<0.05)。结论复发抑郁症患者存在额叶结构和神经认知的改变,结构改变与认知损害存在一定的相关性。Objective To explorer the relationship of cognitive function and the frontal gray matter concentration (GMC) in female major depression. Methods Thirty female major depressed patients (single episode 14, recurrent 16) and age-, education-matched 16 healthy female controls underwent conventional and structure magnetic resonance imaging (3D), and a standardized neuropsychological test to estimate executive function. Results There was no significant difference in GMC between first episode depression and healthy control, and between depressed groups ( P 〉 0. 05 ). Recurrent depressed patients showed reduced GMC in the bilateral superior frontal Gyrus (P = 0. 03, P = 0. 02 ) and right middle frontal Gyrus (P = 0. 02 ) compared with healthy controls. The executive function in recurrent depressed patients were also worse compared with healthy controls ( F = 3.81 - 11.18, all P 〈 0. 05 ). The inferior frontal lobe GMC were negatively with the response errors, response perseverative errors in recurrent depression ( r = - 0. 51, P 〈 0. 05 ; r = - 0. 53, P 〈 0. 05 ). Conclusions Recurrent depressed patients have abnormalities in frontal GMC and cognitive function, and the gray matter reduction of frontal is associated with psychopathology and cognitive dysfunction.
