天然型无机骨植入治疗22例慢性根尖周病变

摘　　要：目的：观察前牙、双尖牙慢性根尖周病变骨质破坏时，植入天然型无机骨（ＮＮＢ）的修复与愈合效果。方法：通过外科手术切除２２例患者（２２颗牙）的根尖周病变，在骨缺损区植入ＮＮＢ。结果：２２例患者中治愈１９例，牙龈瘘管消失，３个月～１２个月时Ｘ线片显示ＮＮＢ颗粒间的点隙状阴影逐渐变浅、模糊，缺损区边缘ＮＮＢ颗粒与骨床融为一体，缺损被完全修复。２例术后伤口裂开，有ＮＮＢ颗粒漏出，经再次缝合创口Ⅱ期愈合。１例术后瘘管复发，拔除患牙。结论：ＮＮＢ组织相容性好，无免疫排异反应，有良好的引导成骨能力。该治疗方法简单，疗程短，特别适合前牙与双尖牙的根尖周病变。Objective:To observe the restoration and healing effects of implantation of natural non organic bone(NNB)in chronic periapical bone destruction of anterior teeth or bicuspid teeth.Methods: 22 cases of periapical diseases were studied.After surgical removal the periapical pathological tissue,NNB was implanted in the bone destructive region.Results:19 cases were cured completely,the gingival fistula was disappeared,the X ray taken 3 12 months later demonstrated that the spot shadow of NNB granules were confused and the marginal bone of the destruction was coalesced with the NNB granules,the destruction was restored completely.In two cases,the wounds were dehiscent postoperatively,some NNB granules leaked,and these two cases underwent because of recurrence of curettage and resuture operation.The bad tooth of one case was taken out because of recurrerce of the gingival fistula.Conclusion:NNB have a good histocompatibility without immunologic rejection,and have greater conductive osteogenetic ability.This therapy is simple and may shorten the course of the treatment.The chronic periapical diseases of the anterior teeth or bicuspid teeth is a special indication.
