摘 要:目的观察食物低血糖指数(LGI)结合低血糖负荷(LGL)在2型糖尿病(T2DM)防治中的应用及效果评价。方法从上海市卢湾区6个居委会480名中老年基线调查对象中,筛选出符合入选和剔出标准的41例T2DM(T2DM组)和32例IFG(IFG组)患者,通过社区分期授课、编制个体化的LGI和LGL食谱、入户指导、电话咨询等方式对其进行为期6个月的营养干预。结果干预后,两组空腹血糖、糖化血红蛋白、总胆固醇、甘油三酯及丙二醛含量均显著下降(P<0.01),血清总超氧化物歧化酶水平显著升高;能量、营养素及LGI食物的摄入有显著改善(P<0.05或0.01),平均一日膳食总GI和GL值显著降低(P<0.01);对所授知识的知晓率显著增高(P<0.01)。结论基于LGI和LGL相结合的营养干预模式易被T2DM及IFG患者接受,有利于患者控制血糖血脂水平、提高抗氧化能力及改善营养摄入状况。Objective To observe the effects of nutrition intervention based on low food glycemic index (LGI)and glycemic load( LGL )on blood glucose, lipid profiles in patients with type 2 diabetes mellitus (T2DM) and impaired fasting glucose (IFG). Method Among 480 middle-aged objects in a baseline survey of six Shanghai Luwan District's communities, 45 patients with T2DM (group DM) and 32 patients with IFG (group IFG) were enrolled and received a six-month nutrition intervention by the ways of community instruction, individual LGI and LGL recipes, door-to-door guidance and telephone consultation. Results After intervention, in the two groups, fasting blood glucose, glycosylated hemoglobin, total cholesterol, triglycerides and malonaldehyde were decreased significantly, while SOD increased; The intake of energy, nutrients and low glycemic food improved. Average daily dietary GI and GL were decreased significantly. The percent of correct answer to nutrition questions was increased significantly. Conclusion The nutrition intervention model based on LGI and LGL is not only easily accepted by patients with T2DM and IFG, but also beneficial to improving their blood glucose, lipid profile, antioxidation capacity and nutritional state.
