摘　　要：目的总结经单侧鼻腔蝶窦入路切除垂体腺瘤的手术经验及疗效。方法回顾性分析30例垂体腺瘤患者的临床表现及手术方法和经验。结果肿瘤全切除22例,次全切除6例,部分切除2例。术后视力及视野改善者20例,内分泌指标恢复正常者26例。视力及视野无明显改善者2例。2例出现顽固性尿崩3月后治愈。3例脑脊液鼻漏2周后治愈。本组无死亡病例。结论经单侧鼻腔蝶窦入路切除垂体腺瘤手术入路直接、操作时间短、创伤小、并发症少,是较理想的经蝶手术术式。Objective To summarize the mononostril-transsphenoidal approach for pituitary adenoma.Methods The clinical features,operative techniques,and outcome of 30 patients with pituitary adenoma were analyzed retrospectively.Results Tumors were totally removed 22 cases,and near-totally resected in 6 cases,and partially resected in 2 cases.Among these cases,20 patients recovered well and their sight was improved and fields of vision expanded.26 patients internal secretion index became normal.2 patients＇ sight and vision field have not been improved obviously. 2 patients got diabetes insipidus but they were cured after 3 months. 3 cases of cerebrospinal fluid stillicidium narium recovered 2 weeks later. There were no deaths in this group. Conclusion The mononostril -transsphenoidal approach for pituitary adenoma is a effective transsphenoidal surgical approach. It has many advantages, such as direct approach, short operative time, minimal invasion, and few complications.
