摘 要:目的检测肺癌中DNA含量,以探讨其在肺癌诊断中的价值。方法应用流式细胞术分别检测35例肺癌患者(病例组)及14例良性肺病变者(对照组)的DNA指数、异倍体率和增殖指数,分析DNA指数、异体倍率、增殖指数与肺癌组织学类型、分化程度、TNM分期等的关系。结果①35例肺癌中29例出现DNA异倍体,4例为多异倍体,肺癌DNA异倍体阳性率为83%,多异倍体率为11.4%,病例组异倍体阳性率远高于对照组(14%),诊断肺癌的敏感性、特异性分别为83%、90%。病例组与对照组相比,DNA指数、增殖指数、异体倍率差异有统计学意义(P〈0.05)。②对比异倍体肺癌组与二倍体肺癌组,DNA指数差异有统计学意义(P〈0.01),增殖指数差异无统计学意义。③DNA指数、增殖指数、异倍体率与病理类型、TNM分期、分化程度及淋巴结转移无关。结论以流式细胞术检测肺癌DNA含量来诊断肺癌,其敏感性、特异性均高,可作为诊断肺癌的很有价值的手段。检测肺癌DNA含量,仅从总量上反映肺癌遗传物质的变化,不能用来区分病理类型、TNM分期、分化程度及有无淋巴结转移。Objective To study the value of DNA content in the diagnosis of lung cancer. Methods DNA index(DI), the rate of heteroploid(RH) and proliferation index(PI) were determined by flow cytometry in 35 lung cancer specimens and 14 matched benign specimens, and analyzed with histological tumor type, differentiation and tumor stage. Results (1)Among 35 lung cancer patients,29 cases were found aneuploidy and 4 cases were determined multiploid. Aneuploidy pattern was much higher in lung cancer group (83 %) than that in control group (14%). Analysing of DNA content as one of diagnostic method in the study, sensitivity and specificity could reach 83 % and 90%, respectively. There was statistical difference on DI, RH and PI between lung cancer group and control group. (2)Compared with diploidy lung cancer patients, DI was much higher in aneuploidy lung cancer patients. However, there was no significant difference on PI. (3)The results showed that DI, RH and PI were not correlated with histologie type,tumor stage,differentiation and lymph node metastasis in lung cancer. Conclusions Determination of DNA content by flow cytometry can be used as valuable method in diagnosis of lung cancer due to its high sensitivity and specificity. Flow cytometry can only determine total quantity of patients' genetic material, but it fails to discriminate different histologic type, tumor stage,differentiation and lymph node metastasis in lung cancer.
