常规放疗与准适形放疗治疗鼻咽癌的放射反应对比研究

摘　　要：目的对常规放疗与准适形放疗治疗鼻咽癌的放射反应对比研究,探讨准适形放疗方法的科学性及实用性。方法将156例经病理确诊的初治鼻咽癌患者随机分为常规组和准适形组,每组78例,准适形组采用面膜固定、模拟定位、摄片、勾画靶区、制作铅挡块、等中心外照射;常规组行常规放疗。观察两组近期疗效及早期急性放射性反应。结果两组近期疗效比较,差异无统计学意义(P>0.05);早期急性放射性反应如口腔黏膜反应、皮肤反应、胃肠道反应的发生率准适形组低于常规组,差异有统计学意义(P<0.05);两组耳、咽、唾液腺反应的发生率比较,差异无统计学意义(P>0.05)。结论准适形放疗通过鼻咽癌病人的个体化设计,达到对鼻咽照射野的精确适形照射,邻近重要组织器官得到有效遮挡,既不影响治疗疗效又减轻放射反应,提高了患者治疗的依从性及生活质量。Objective To compare the effect between Conventional radiotherapy and quasi-conformal radiotherapy in patients with nasopharyngeal carcinoma（NPC）,illuminate quasi-conformal radiotherapy is scientific,practical,and precise.Methods 156 cases with nasopharyngeal cancer were randomly divided into the conventional radiotherapy group and the quasi-conformal radiotherapy group.Both of two groups received radiation with X-ray,a radical doses of 66-70GY in the nasopharynx.The two groups were observed short-term efficacy and early acute radiation reaction.Results The difference of curative effect in the near future between the two groups was not significant statistically（P〉0.05）.With respect to the acute radiotion induced reaction,the reaction of gastromtestinal and mucous of oral cavity was milder in the observation group than that in the control group（P〈0.01）.Conclusion The quasi-conformal radiotherapy will decrease the side effects of radiation by protecting adjacent tissues and organs,improve patients＇s treatment compliance and quality of life.
