摘  要:目的探讨磁共振弥散成像技术对肝脏肿瘤性病变的临床诊断价值。方法对30例肝囊肿、30例肝脏海绵状血管瘤、37例原发性肝癌、14例转移瘤患者行T1WI、T2WI、动态增强扫描和弥散加权扫描,分别进行病变实性区域及正常区域表观弥散系数(ADC值)的测量并进行分析。结果肝囊肿、肝血管瘤与肝癌及转移瘤患者病变区域的实性部位ADC值分别为(2.87±0.57)×10-3mm2/s、(1.99±0.31)×10-3mm2/s(、1.28±0.23)×10-3mm2/s、(1.17±0.13)×10-3mm2/s、肝癌与肝囊肿、肝血管瘤差异有统计学意义;肝癌与转移瘤患者病变区域的实性部位ADC值差异无统计学意义(P=0.276)。结论磁共振弥散成像技术对肝脏肿瘤性病变的诊断与鉴别诊断有一定的实用价值,实性恶性肿瘤的鉴别仍需进一步探讨。Objective To investigate the diagnosis value of diffusion weight imaging(DWI) in liver masses. Methods Routine MR T1WI,T2WI,dynamic enhanced MR and diffusion weighted images were obtained in 30 cysts,30 hemangiomas,37 hepatocellular carcinomas and 17 metastatic tumors in livers. The characteristics of the DWI and apparent diffusion coefficiene(ADC) value were analyzed. Results The ADC value of hepatocellular carcinomas, cyscs and hemangiomas was ( 1.28 ± 0.23) ×10^-3 mm^2/s, (2.87 ±0.57) ×10^-3 mm^2/s, ( 1.99±0.31 ) ×10^-3 mm^2/ s respectively and showed significant difference among three group. The ADC value was not significantly different between hepatoeellular carcinomas and metastatic tumors ( P 〉 0.05). Conclusion The measurement of ADC values is helpful in MR diagnosis and differentiation of liver masses. It still need to explore more in differentiation of malignant tumors.
