摘 要:目的探讨DWI在幕上高级别胶质瘤与脑转移瘤鉴别诊断中的应用价值。方法收集在治疗前行常规MRI及DWI检查的幕上脑肿瘤共45例,其中高级别胶质瘤(HI~Ⅳ级)18例,脑转移瘤27例。以瘤周1cm为界线,将瘤周水肿区划分为近瘤区及远瘤区,分别测量高级别胶质瘤和脑转移瘤瘤周水肿区的ADC值,并计算rADC值。结果高级别胶质瘤近瘤区rADC值明显低于脑转移瘤近瘤区rADC值,差异有统计学意义(P〈O.01),二者远瘤区rADC值间无显著性差异(P〉0.05)。结论近瘤区rADC值有助于幕上高级别胶质瘤与脑转移瘤的鉴别诊断。Objective To discuss the diagnostic value of DWI in differentiating supratentorial high-grade gliomas from brain metastases. Methods 45 patients with supratentorial tumors including 18 cases of high-grade ( Ⅲ ~ IV ) gliomas and 27 cases of metastases were involved in this study. All patients underwent conventional and diffusion weighted MR imaging. The peritumoral edema regions were divided into adjacent peritumoral region and far peritumoral region with a borderline, which had 1 centimeter distance from the enhancing edge of the tumor. The ADC values in different peritumoral edema'regions of high-grade gliomas and metasta- ses were measured respectively, and the rADC values were calculated . Results In adjacent peritumoral edema region , the rADC values of high-grade gliomas were significantly lower than that of matastases , and the difference was statistically significant (P〈 0.01). The difference of rADC values in far peritumoral edema between high-grade gliomas and metatases was not statistically significant (P〉0.05). Conclusion The rADC values of adjacent peritumoral edema region are helpful in distinguishing supratentorial high-grade gliomas from metastases.
