腹腔镜下术中超声介入微波消融治疗浅表肝癌的临床探讨

摘　　要：目的探讨腹腔镜下术中超声介入微波消融治疗浅表肝癌的疗效。方法回顾分析11例肝癌患者共15个浅表病灶在腹腔镜下术中超声引导穿刺冷循环微波消融治疗的效果。结果13个病灶术后一个月经超声造影和增强CT或增强MRI复查,病灶区内未见血流信号及强化,2个病灶术后一个月复查病灶区内可见少许血流信号及强化,进行第二次治疗,其后一个月复查病灶区内未见血流信号及强化。现10例患者健在,生存期在3～22个月之间,仍在随访之中。结论腹腔镜下术中超声介入微波消融治疗浅表肝癌的疗效良好,体现了微创外科观念,拥有巨大的发展潜力。Objective To discuss the therapeutic effects on microwave ablation treatment of superficial liver cancer guided with laparoscopie intra-operative ultrasound. Methods 11 patients with 15 lesions were retrospectively analyzed. All cases were treated by microwave ablation guided with laparoscopic intra-operative ultrasound. Results Contrast-enhanced ultrasound and contrast-enhanced CT/MR were performed on 11 patients one month after treatment. No blood signal or enhancement was detected in 13 lesions. Little blood signal or enhancement was detected in 2 lesions. The 2 lesions underwent a second treatment and no blood signal and enhancement were found within fociarea after one month re-examination. 10 patients were still alive, with a survival period of 3-22 months. The patients were still followed-up. Conclusion The therapeutic effects on microwave coagulation treatment of superficial liver cancer guided with laparoscopie intra-operative ultrasound is good method. Microwave ablation treatment guided with laparoscopic intra-operative ultrasound is a minimally invasive method with great potential.
