彩色多普勒超声检查食管癌锁骨上淋巴结与放疗反应的关系

摘　　要：目的:观察彩色多普勒超声检查在锁骨上淋巴结中的应用及与放疗疗效的关系。方法:对90例食管癌患者行双侧锁骨上淋巴结区彩超检查,触诊及彩超检查(+)者行针吸细胞学检查并测阻力指数(RI值)。结果:直径≥1cm者针吸细胞学检查阳性率为65.9%(27/41),高于<1cm者的38.1%(8/21)(χ2=4.353,P<0.05)。RI≥0.7者针吸细胞学检查阳性率为60.8%(31/51),高于RI<0.7者的30.4%(7/23)(χ2=7.459,P<0.05)。RI≥0.7者放疗反应阳性率为68.6%(35/51),高于RI<0.7者的34.8%(8/23)(χ2=5.845,P<0.05)。结论:彩超可作为锁骨上淋巴结的常规检查,有助于疗效评价。Aim：To explore the value of color Doppler flowing imaging in the examination of supraclavicular lymph node of esophageal neoplasm and the result of radiotherapy.Methods：Ultrasound was employed for 90 patients in supraclavicular area of esophageal neoplasm patients.And 74 patients accepted fine needle biopsy and rheographic indext（RI） were tested.Results：The positive rate of needle biopsy was higher in lymph node diameter ≥1 cm group as compared with ＆lt;1 cm group（65.9% vs.38.1%）（χ^2=4.353,P〈0.05）,in RI≥0.7 group as compared with 〈 O. 7 group（60.8% vs.30.4% ） （χ^2 = 7. 459 ,P 〈 0.05 ）. The positive response rate of radiotherapy in RI≥0.7 and 〈 0.7 groups were 68.6% and 34.8% （χ^2= 5. 845 ,P 〈 0.05 ）. Gonclusion ： Color Doppler flowing imaging is important to rule out supraclavicular lymph node metastasis, and is value to evaluate the result of radiotherapy.
