摘 要:目的:探讨同一个体食管、贲门双源癌的人乳头状瘤病毒(HPV)感染和p53、p16蛋白表达与食管和贲门癌变的关系。方法:采用聚合酶链式反应(PCR)检测17例双源癌食管鳞癌组织和贲门腺癌组织人乳头状瘤病毒16型(HPV16)感染特征,采用免疫组织化学方法检测p53和p16蛋白在17例双源癌鳞癌组织和腺癌组织中的表达。结果:17例双源癌组织中8例食管鳞癌组织,5例贲门腺癌组织中检测到HPV16-DNA(47%对29%),且有2例患者食管鳞癌和贲门腺癌组织中同时检测到HPV16-DNA的存在。p53和p16蛋白在两种组织中均有较高表达。p53蛋白突变率在HPV阳性组织中均较HPV阴性组织低(50%对77%;60%对66%);在HPV相关的双源癌中,p16蛋白表达在腺癌组织中较鳞癌组织高(75%对25%)。结论:同一个体食管鳞癌和贲门腺癌均有不同程度HPV感染,提示HPV可能是两者共同相关致病危险因素,高危型HPV16感染可能在食管鳞癌和贲门腺癌发生中起重要作用。p16蛋白表达在腺癌组织中较鳞癌组织高,有可能成为HPV相关性贲门腺癌的一个筛查指标。Objective: To investigate the correlations of HPV16 infection and p53, p16 expression in concurrent esophageal and gastric cardia carcinogenesis from same patients at high-incidence area in Henan. Methods: HPM16-DNA was deteded by polymerase chain reaetion(PCR),expressions of p53 and p16 were detected by ABC immunohistochemistry in 17 surgically reseeted specimens from the concurrent esophageal and gastric cardia cancers(CC). Results: The positive rate of HPV16E6-DNA was 47% (8/17) in esophageal squamous cell careinoma(ESCC), 29% (5/17) in gastric eardia adenoeareinoma(GCA), respectively. And HPV16E6- DNA was observed simultaneously in 2 ESCC and 2 GCA. The expression of p53 and p16 protein were observed both in ESCC and GCA tissues with different degrees. The expression rates for p53 in HPV positive and negative tissues were 50%,77% in ESCC, and 60%,66% in GCA, respectively. The expression of p16 protein was significantly higher in GCA than that in ESCC(75 % vs 25 %, P〉0. 05). Conclusion:The consistent HPV infection in CC from the patients at same area suggest that HPV may be one of the common oncogenic factor for both ESCC and GCA. HPV16, the high-risk type, may play an important role in the development of ESCC and GCA. p16 may be another candidate biomarker for screening of HPV-related GCA.
