p53、CEA在胃黏膜上皮内瘤变及胃癌中的表达及其意义

摘　　要：目的探讨p53、癌胚抗原(CEA)在胃黏膜上皮内瘤变及胃癌中的表达及其临床意义。方法应用免疫组织化学方法(S-P法)检测40例正常胃黏膜、68例胃黏膜低级别上皮内瘤变、60例胃黏膜高级别上皮内瘤变以及80例胃癌中p53、CEA的表达情况。结果正常胃黏膜组p53、CEA均为阴性表达;p53在胃黏膜低级别上皮内瘤变组阳性表达率为17.65%,明显低于胃黏膜高级别上皮内瘤变组(40.00%,P<0.05)和胃癌组(56.25%,P<0.05)。CEA在胃黏膜低级别上皮内瘤变组阳性表达率为36.76%,明显低于胃黏膜高级别上皮内瘤变组(68.33%,P<0.05)和胃癌组(82.50%,P<0.05)。p53、CEA共同表达在胃黏膜低级别上皮内瘤变组为14.70%,明显低于胃黏膜高级别上皮内瘤变组(31.67%,P<0.05)和胃癌组(45.00%,P<0.05)。结论p53和CEA与胃癌关系密切,对胃黏膜上皮内瘤变联合检测p53和CEA,有助于发现早期胃癌。Objective To investigate the role ofp.53 and CEA in gastric intraepithelial neoplasia and gastric carcinoma. Methods The technique（ SP method） was used to detect the expressions of p53 ,CEA in 40 cases of normal gastric mucosa,68 cases of low grade intraepithelial neoplasia in gastric mucosa（LGIN）, 60 cases of high grade intraepithelial neophsia in gastric mucesa（ HGIN）, 80 cases of gastric carcinoma（GC）. Results No positive expressions of p53 and CEA proteins were found in normal gastric mucesa cases. The positive rate of p53 in LGIN （ 17.65 % ）was lower than that in HGIN （40. 00%, P 〈 0. 05 ）and GC （56. 25 %, P 〈 0. 05 ）. The positive rate of CEA in LGIN （ 36. 76% ） was lower than that in HGIN （68. 33 %, P 〈 0. 05 ） and GC （ 82. 50%, P 〈 0. 05 ）. The co-positive rate of p.53 and CEA in LGIN （ 14. 70% ） was lower than that in HGIN （ 31.67%, P 〈 0. 05 ） and GC （45.00%, P 〈 0. 05 ）. Conclusions The expressions of p53, CEA are correlated with HGIN and GC. Combined detection of p53 and CEA in gastric intraepithellal neoplasia may be useful for diagnosis of early gastric carcinoma.
