Twist及E—cadherin在甲状腺乳头状癌中的表达及其临床病理意义

摘　　要：目的探讨Twist、E—cadherin在甲状腺乳头状癌中的表达和相互关系及其临床病理意义。方法应用免疫组化方法，对印例甲状腺乳头状癌、40例结节性甲状腺肿及40例良性乳头状病变进行Twist、E—cadherin表达的研究。结果甲状腺乳头状癌Twist及E—cadherin阳性表达率分别为81．67％（49／60）及21．67％（13／60），结节性甲状腺肿Twist及E—cadherin阳性表达分别为0％（0／40）及87．50％（35／40），良性乳头状病变Twist及E—cadherin阳性表达分别为7．5％（3／40）及100％（40／40），前者与后两者相比差异有统计学意义（P〈0．05）；Twist及E—Cadherin在甲状腺乳头状癌中的表达水平与病人的年龄及原发肿瘤分期无关，但与有无淋巴结转移相关（P〈0．05）；Twist与E—cadherin的表达呈负相关（r=-0．796，P=0．000）。结论在大多数甲状腺乳头状癌的上皮细胞中都存在Twist过表达及E—cadherin表达下调，其淋巴结转移的发生与Twist过表达及E—cadherin的表达减少或功能的不正常可能有密切的关系；Twist与E—cadherin的表达呈负相关。Objective To study the expression of Twist, E -cadherin in Papillary thyroid carcinoma （PTC） and the relationship with chnicpathological factors. Methods Twist and E - cadherin were measured in papillary thyroid carcinomas （ n = 60）, nodular goiter（ n = 40）, benign papillary lesions （ n = 40） by immunohisto- chemical staining. Resusts In papillary thyroid carcinoma（ PTC）, positive rates of Twist and E - cadherin were 81.67% （49/60） and 21.67% （13/60） ; in nodular goiter（ NG）, positive rates were 0% （0/40） and 87.50% （ 35/40 ） ; in benign papillary lesions （BPL）, positive rates were 7.5 % （ 3/40 ） and 100% （ 40/40 ）, respectively. There were statistic significance compared PTC with NG and BPL. The positive expression of Twist and E - cadherin were both related to lymph node metastasis. The expression of Twist showed negative correlation with E - cadherin. Conclusions Twist expression in PTC was increased and E - cadherin was reduced. They were both correlated with lymph node metastasis. The expression of Twist showed negative correlation with E - cadherin.
