Preliminary study on detection of hypoxic brain tissue around the focus of cerebral hemorrhage using HL-91 single photon emission computerized tomography

摘  要:目的探讨乏氧组织显像剂(99Tcm-HL-91)单光子计算机断层扫描(SPECT)检测脑出血灶周围乏氧组织的应用价值。方法应用HL-91SPECT脑显像对26例脑出血患者进行检测,并与CT影像学结果及5名健康对照者进行比较。结果26例脑出血患者中17例(65.38%)乏氧显像阳性,乏氧显像阳性区与CT上脑出血灶密切相关;9例阴性者中,6例为小量脑出血患者,包括2例出血量<10ml的小脑出血和丘脑出血,4例出血量<15ml的基底节区和脑叶出血者,另3例为起病后20d检查者。对照组全部阴性。结论HL-91SPECT脑乏氧显像完全适用于脑出血后乏氧组织的临床观察。HL-91SPECT检测所显示出的乏氧区与CT上的脑出血灶的密切相关。Objective To investigate the applied value of ^99Tc^m-HL-91 single photon emission computerized tomography(SPECT) in detecting the hypoxie brain tissue around the focus of cerebral hemorrhage. Methods 26 patients with cerebral hemorrhage were examined by HL-91 SPECT. And the results were compared with their skull CT imagings and other 5 healthy controls. Results 17 case (65.38%) of cerebral hemorrhage group were found hypoxie imaging positive, and the hypoxic zone was closely related to the focus of cerebral hemorrhage in CT imaging. 9 eases of cerebral hemmorhage group were found hypoxic imaging negative, 6 cases of which were small quantity cerebral hemorrhage, including 2 cases 〈 10 ml hemorrhage in cerebellar and thalamic, and 4 cases 〈 15 ml hemorrhage in basal ganglia and cerebral lobe ; another 3 negative cases were inspected at 20 d after onset. 5 cases of normal control group were all negative in HL-91 SPECT. Conclusions HL-91 SPECT hypoxic imaging is applicable to the clinical observation of hypoxie brain tissue around cerebral hemorrhage. The hypoxic zone in HL-91 SPECT imaging is closely related to the focus of cerebral hemon-hage in CT imaging.
