后巩膜加固术治疗高度近视黄斑劈裂

摘　　要：目的探讨后巩膜加固术对高度近视黄斑劈裂的治疗效果。方法对15例（20只眼）高度近视黄斑劈裂患者行改进的后巩膜加固术。术后观察患者的最佳矫正视力、屈光度、眼轴长、黄斑区视网膜厚度及手术并发症。结果术后随访期最佳矫正视力较术前明显提高，差异有统计学意义（P〈0．05）。近视屈光度和眼轴长均较术前减少，差异有统计学意义（P〈0．05）。OCT示劈裂的视网膜内外层之间的腔隙有不同程度的缩小，其中有5例劈裂腔完全消失，3例伴有浅脱离的视网膜均达到解剖复位，视网膜厚度较术前显著下降，差异有统计学意义（P〈0．05）。未察及严重并发症的发生。结论对于高度近视黄斑劈裂伴或不伴视网膜脱离的治疗，改进的后巩膜加固术具有较好的可行性和安全性。Objective To investigate the effect of posterior scleral reinforcement for treatment of high myopia macular splitting. Methods Fifteen myopia patients （20 eyes） who had macular splitting underwent posterior scleral reinforcement surgery. Main outcome measures included best-corrected visual acuity,refractive error,axial length,optical coherent tomography findings and surgery complications. Results Best corrected visual acuity improved significantly after surgeries in the following-up period （P 〈 0.05）. Myopic diopter and axial lengths also reduced significantly （P 〈 0.05）. OCT showed the split-cavity between the inner and outer retinal layer reduced in different degrees, the thickness of the retina declined significantly （P 〈 0.05 ）. No serious complications were observed. Conclusions Posterior scleral reinforcement surgery is effective and safe for treatment of high myopia macular splitting with or without retinal detachment.
