摘 要:目的利用光学相干断层扫描仪测量近视眼患者视网膜神经纤维层(RNFL)厚度,分析其分布特点及与屈光度的关系。方法连续非随机选择91例眼科门诊近视患者(91只眼)纳入研究,其中低度近视(等效球镜度数〉-3.0D)28例(28只眼),中度近视(等效球镜度数-0.3~-6.0D)33例(33只眼),高度近视(等效球镜度数〈-6.0D)30例(30只眼)。均接受包括眼压、验光、裂隙灯、眼底等常规眼部检查,排除除屈光不正以外其他眼部疾病。利用RTVue Fourior-OCT测量视盘周围RNFL厚度。屈光度由等效球镜度数表示。数据分析采用SPSS13.0软件处理。结果近视眼患者盘周平均神经纤维层厚度(108.5-±10.1)μm,其中下、上方位较厚,鼻侧最薄。单因素相关及多元线性回归分析发现:鼻侧偏上、偏下和下方偏鼻侧区域RNFL厚度与等效球镜度数具有显著性正相关(P〈0.05);下方偏颞侧RNFL厚度与等效球镜度数成显著性负相关(P〈0.05)。而其他部位RNFL厚度与等效球镜度数无相关。结论平均RNFL厚度与近视度数无相关性,只有鼻侧及鼻下方厚度随近视度数增加而减小,而颞下方随近视度数增加而增加,由于与青光眼RNFL损伤特点不同,因此RNFL测量可以作为在近视患者中检测和评价青光眼视神经损害的有用的指标。Objective To assess the relationship between retinal nerve fiber(RNFL) thickness measured by optic coherence tomography and refractive diopter. Methods 91 cases (91 eyes) selected continually and unrandomly from ophthalmological outpatients including 28cases with low myopia (spherical equivalence [SE] 〉-3.0D), 33 cases with moderate myopia (SE-0.3D - -6.0D ) and 30 cases with high myopia(SE〈-6.0D). All patients received ocular standard examination including intraocular pressure,refraction, slitlamp biomicroscopy and fundas. Other ocular diseases except refractive error were excluded. RNFL thickness was measured by RTVue Fourior-OCT. Refraction diopter was shown as SE. All data were analysed by SPSS13.0 software. Results The mean RNFL thickness was 108.5± 10.1μm,and inferior and superior RNFL were thickest and nasal was thinnest. RNFL thickness of up and down of nasal and inferior region were significant positive correlative with SE (〈0.05), those in tempory of inferior region were significant negtive correlative with SE (〈0.05) ,and those in other regions were not significant correlative with SE (〉0.05) in single and multiple variable regressive analysis. Conclusion Mean RNFL thickness may not corrective,the up and down of nasal and inferior region may decrease,and tempory inferior region may increase with increase myopia diopters. RNFL thickness may be an useful parameters during detect and assess the glaucomatous neuropathy since the different of RNFL changes in eyes with myopia and glaucoma
