自体口腔黏膜上皮细胞培养眼表重建的实验研究

摘　　要：目的将自体培养的口腔黏膜上皮植片移植于重度碱烧伤后角膜缘功能完全缺乏的动物眼表，分期观察上皮恢复情况及其新生血管的生长情况。方法建立单眼重度碱烧伤模型．，随机分为对照组（不行移植）、单纯羊膜移植组、自体培养的口腔黏膜细胞移植组。术后每日在裂隙灯下观察角膜上皮染色、角膜混浊度以及新生血管生长情况并进行评分；测定角膜新生血管（CNV）长度，计算CNV面积；测量角膜微血管数量；检测血管内皮细胞生长因子（VEGF）表达；利用印迹细胞技术观察角膜上皮重建情况。结果口腔黏膜上皮细胞移植组总评分、CNV面积、微血管计数、VEGF阳性表达量均最少。印迹细胞检查：口腔黏膜细胞移植组角膜上皮未发现杯状细胞。结论利用体外培养的自体口腔黏膜上皮细胞进行移植，可减少角膜新生血管的形成，其抑制新生血管的作用优于单纯羊膜移植抑制新生血管的作用。Objective Animal ocular surface was destroyed by severe alkali which led to corneal limbal stem cell deficiency. Transplant autologous cultivated mucosal epithelial sheet on the animal ocular surface to observe the situation of epithelium recovering and corneal neovascularization. Methods Models of single eye burned by severe corneal alkali were made and divided them into control group without transplantation, amniotic membrane （AM） transplanted group, autologous cultivated mucosal epithelial transplanted group. Therapeutic effect was evaluated by the total scoring of corneal fluorescence staining, opacity and corneal neo- vascularinzation which were observed with a slit lamp microscope. Calculate the length and the area of CNV, the number of capillary vessels. The expression of vascular endothelial factor was measured. Corneal impression cytology was performed to assess the phenotype of corneal epithelium. Results The total scoring of cornea, the area of CNV, the number of capillary vessels and the expression of VEGF in autologous cultivated mucosal epithelial transplanted group were less than other groups. And goblet cells in this group were not discovered. Conclusions The transplantation of autologous cultivated mucosal epithelial sheet on the animal ocular surface can inhibit CNV, and the effect of the inhibition of CNV in this group was larger than that in AM group.
