摘 要:目的研究胰腺癌中COX-2表达的意义。方法应用免疫组化SP法检测82例胰腺癌、22例慢性胰腺炎、9例胰腺良性肿瘤、15例正常胰腺组织和2株人胰腺癌细胞中COX-2的表达,然后比较胰腺癌组织COX-2表达与胰腺癌患者临床病理特征的关系。结果2株人胰腺癌细胞COX-2表达均阳性。70.7%(58/82)胰腺癌组织可见COX-2表达,而正常胰腺组织只有6.7%(1/15)呈微弱表达。COX-2在4种组织中的表达程度有显著性差异(P<0.001)。胰腺癌组织COX-2表达显著高于其他3种组织(P<0.05)。胰腺癌组织COX-2表达水平的高低与胰腺癌的临床病理特征无关(P>0.05)。结论COX-2蛋白的检测对胰腺癌的诊断及其与胰腺良性肿瘤、慢性胰腺炎的鉴别诊断有帮助。胰腺癌组织COX-2表达不能作为预测患者预后的指标。Objective To investigate the significance of cyclooxygenase-2 (COX-2) expression in pancreatic cancer. Methods COX-2 expression was measured in tissue specimens from 82 patients with pancreatic cancer, 22 chronic pancreatitis, 9 benign pancreatic tumors and 15 normal pancreatic tissues, and 2 human pancreatic cancer cell lines using immunohistochemistry. COX-2 expression in pancreatic cancer tissue and elinicopathological features were compared. Results COX-2 expression was found in 2 human pancreatic cancer cell lines. COX-2 expression was found in 70.7% (58/82) of patients with pancreatic cancer, but it was not detected or weakly expressed in 6.7% (1/15) of normal pancreatic tissues. There was significant difference of COX-2 expression among the above four groups (P 〈 0. 001 ). COX-2 expression in pancreatic cancer was significantly higher than that in other three tissues (P 〈 0.05). COX-2 expression in pancreatic cancer tissue showed no correlation with clinicopathological features of patients with cancers (P 〉 0.05). Conclusion COX-2 over-expression might play an important role in the carcinogenesis and development of pancreatic cancer. The determination of COX-2 expression might be valuable for the diagnosis of pancreatic cancer and differential diagnosis of pancreatic cancer from benign pancreatic tumors and chronic pancreatitis. The COX- 2 expression in pancreatic cancer tissue could not be used as a parameter for predicting the prognosis of the patient. COX-2 inhibitors might be potentially effective for prevention and treatment of pancreatic cancer.
