摘 要:目的:以新型染色剂叶绿素显示液(chlorophyll disclosing solution,CDS)活体注射,研究正常小鼠直肠淋巴引流途径及染色淋巴结的超微结构,为直肠癌的淋巴转移提供参考依据,并为叶绿素在直肠癌淋巴结清扫中的染色应用提供理论参考。方法:24只成年鼠随机分为实验组(A组,活体直肠壁内注入CDS)和空白对照组(B组)。观察A组小鼠不同时间点染色淋巴结位置和数目,两组皆清点直肠各级淋巴结个数,淋巴结固定、切片,染色,光、电镜观察。结果:于直肠壁内注射CDS,髂动脉间淋巴结首先着色,其次为髂动脉旁淋巴结,然后为肾门淋巴结。淋巴结染色迅速,染色剂不易沉淀,染色淋巴结与周围组织对比明显:A组平均每鼠检出淋巴结(6.92±1.00)个,B组平均每鼠检出淋巴结(4.75±0.87)个,A组显著高于B组(P<0.01):染色24 b后淋巴结后仍无褪色,且染色后细胞超微结构无病理改变,术中和术后均未发现不良反应和并发症。结论:(1)经叶绿素染色显示正常小鼠直肠淋巴引流途径为:髂动脉问淋巴结—髂动脉旁淋巴结—肾门淋巴结。(2)叶绿素是一种优良的淋巴结显色剂,提示其可用于直肠癌根治术中指导淋巴结清扫。Objective: To study the rectal lymph draining way of normal mouse and ultrastructure of colored lymphoid for reference of rectal carcinoma lymphatic metastasis way, and for theoretical reference of removing lymphoid in rectal carcinoma operation. We injected in vivo a neotype of stain: chlorophyll disclosing solution (CDS). Methods: 24 adult mice were divided into study group (group A, injecting CDS into the rectal wall in vivo) and black control group (group B) by random. To group A, we studied the position and number of colored lymph nodes in different time. To both of the two groups we counted and collected every grade lymph nodes of rectum. The lymph nodes were fixed, sliced and stained to be observed by light microscope or electron microscope. Results: After injecting CDS in rectal wall, the first colored lymph nodes were interarterioiliac nodes, and the next were lateral iliac artery, nodes, then the Renal hilar nodes. CDS colored lymph nodes quickly and was not easy to precipitate. The dyeing lymph nodes contrasted sharply with surrounding tissues. In group A, the average number of lymph nodes observed in each mouse was 6.92±1.00, and in group B, it was 4.75±0.87. As group A is more than that of group B significantly (P〈0.01). The colored lymph nodes didn't fade after injecting CDS 24 hours. The ultrastmcture of the colored lymph nodes at different interval is no significant difference to the ones uncolored. No adverse reaction or complication was observed in the course of injection and after injection. Conclusion: (1) By injecting CDS in vivo, we found the rectal lymph draining way of normal mouse was: the interarterioiliac nodes the lateral iliac artery nodes - the Renal hilar nodes. (2)The chlorophyll was an ideal dye for staining lymph nodes. As showing that CDS could be used to guide clearance of lymph nodes in radical resection of rectal carcinoma.
