首发精神分裂症糖脂代谢异常与血清肿瘤坏死因子-α的关系

摘　　要：目的分析首发精神分裂症糖脂代谢紊乱状况及其与肿瘤坏死因子-α(TNF-α)的关系。方法对60例首发精神分裂症患者,测定血糖、血脂,采用稳态模型评估指数(HOMA-IR)评估胰岛素抵抗(IR)的发生,同时采用放射免疫法测定TNF-α浓度,并采用阳性和阴性症状量表(PANSS)评定精神病理状况。结果空腹血糖升高、胰岛素抵抗和三酰甘油(TG)升高者分别占16.7%、26.7%和20%。IR者腰围、TNF-α、PANSS总分及N,G分量表得分方面均显著高于非IR者(P<0.05);FBG和IR指数与TNF-α呈正相关(r为0.491和0.263,P<0.05);TG与腰围和BMI均存显著正相关(r=0.372和0.425,P<0.01)。结论首发精神分裂症患者存在不同程度的血糖、血脂异常和胰岛素抵抗,TNF-α与其发生有一定的关系。Objective： To analyze the blood glucose and lipids metabolic abnormalities and the relationship to tumor necrosis factor - α（ TNF - α） in first episode schizophrenia. Methods： Sixty patients with first episode schizophrenia were recruited and the blood glucose and lipids were tested. All subjects were assessed for the presence of insulin resistance （IR） with homeostasis model assessment （HOMA） and TNF -α level was assayed with radicalization immunity. The psychosis status was assessed with the Positive and Negative Symptoms Scales （ PANSS）. Results： The rates of increased fasting blood glucose（FBG） level, IR and increased serum triglyceride （TG） level were 16.7% ,26.7% and 20%, respectively. The index of waistline, serum TNF - α level, the scores of N - subscale , G - subscale and the total score of PANSS of IR group were significantly higher than that of non - IR groups （ P 〈 0. 05 ）. FBG and IR were positively correlated with TNF - α level （ r = 0. 491 and 0.263,P 〈 0. 05 ） and TG was positively correlated with the waistline and BMI （ r = 0. 372 and 0.425, P 〈0.01 ） . Conclusion： There are blood glucose and lipids metabolic abnormalities and IR in first episode schizophrenia. TNF-α may play a role in insulin resistance.
