三维适形放疗治疗非小细胞肺癌的临床观察

摘　　要：目的探讨三维适形放疗治疗非小细胞肺癌的疗效。方法2001年6月至2004年12月采用三维适形放疗的46例非小细胞肺癌患者,并与随机抽取的同期常规放疗的46例非小细胞肺癌患者对照,对比观察两组患者的近期疗效,1、2、3年生存率,中位生存期和毒副反应。结果治疗组对照组的总有效率分别为71.7%和54.3%,其中完全缓解率分别为21.7%和8.7%(P<0.05),两组1、2、3年生存率分别为78.3%、47.8%和29.0%,以及58.7%、34.8%和24.8%。两组患者的中位生存期分别为22个月和16个月。两组毒副反应发生率无显著性差异。结论三维适形放疗治疗非小细胞肺癌的疗效优于常规放射治疗。Objective To evaluate the effect of three-dimensional Conformal radiation therapy（3DCRT） for patients with non-small-cell lung cancer. Methods From June 2001 to December 2004, 46 patients with non-small-cell lung cancer were treated by 3 DCRT. The short-term response and 1-year, 2-year and 3-year survival rates, median survival time and toxicity in 3DCRT group were compared with those in 46 cases with non-small-cell lung cancer received conventional radiation therapy in the same period （control group）. Results The efficacy rates in 3DCRT group and control group were 71.7% and 54.3%., and the complete remission rates were 21.7% and 8.7% respectively, showing a significant difference （P 〈 0.05）. The 1-year,2-year and 3-year survival rates of the patients in 3DCRT group and control group were 78.3% ,47.8% and 29.0% ,58.7% ,34.8%. and 24.8% respectively. The median survival time in 3DCRT group and control group were 22 months and 16 months respectively. The difference of hematological toxicity and acute radiation pneumonitis, esophagitis between the two groups had no statistical significance. Conclusion 3DCRT produces better therapeutic effects than conventional radiotherapy in patients with non-small-cell lung cancer.
