摘 要:目的建立SD大鼠胰腺癌模型,探讨胰腺癌发生过程中胰岛素样生长因子(IGF)-1和其受体(IGF-1R)的表达及其意义。方法将二甲基苯葬蒽(DMBA)直接置入胰腺实质内(A组+B组),B组每周腹腔注射曲古霉素A(TSA)1ug;A组和B组于3~5个月内处死,对照组(C组)于第5个月处死;肉眼检查和HE染色后镜下观察胰腺癌发生;IGF-1和IGF-1R染色方法为常规石蜡包埋切片EnVision^TM免疫组织化学法。结果A组3~5个月癌发生率为48.7%(18/37),17例为胰腺导管癌和1例为纤维肉瘤;B组3—5个月癌发生率为33.3%(12/36),11例为胰腺导管癌和1例为纤维肉瘤;A组胰腺癌最大径均值大于B组(P〈0.05);C组胰腺及A、B、C组胰腺外主要脏器均无明显病理改变;A组+B组胰腺导管癌IGF-1和IGF-1R表达阳性率明显高于A组+B组非癌胰腺组织(P〈0.01),A组胰腺导管癌IGF-1和IGF-1R表达阳性率明显高于A组非癌胰腺组织(P〈0.01),但B组之间差异无统计学意义(P〉0.05);IGF-1和IGF-1R阳性表达的非癌胰腺导管上皮呈轻至重度不典型增生;C组正常胰腺和2例纤维肉瘤IGF-1和IGF-1R均阴性表达。结论较大剂量DMBA置入胰实质内可在短期内获得较高胰腺癌发生率,TSA能抑制胰腺癌的生长,其作用可能与抑制IGF-1和IGF-1R合成和分泌有关。IGF-1和IGF-1R在DMBA诱癌发生过程中可能起着重要促癌作用。Objective To establish a model of pancreatic cancer induced by dimethylbenzanthracene (DMBA) in Spragne-Dawely (SD) rats, and detect the expression levels of insulin-like growth factor- 1 ( IGF-1 ) and its receptor ( IGF-1 R) and their effects on the carcinogenesis of rat pancreas. Methods DMBA was directly implanted into the parenchyma of rat pancreas ( group A and group B ). The rats in group B were treated with 1 ml trichostatin A (TSA) saline solution (1 mg/L) via intraperitoneally weekly. The carcinogenesis of rats executed within 3-5 months in groups A and group B was observed by macrograph and under microscopy. Meanwhile, the rats in the control group ( group C ) were executed in 5 months. The EnVisionTM immunohistochemistry for detecting the expression levels of IGF-1 and IGF-1R was used in conventionally paraffin-embedded sections from above pancreatic specimens. Results ( 1 ) The incidence of pancreatic cancer in group A within 3-5 months was 48.7% ( 18/37 ) ,including 17 cases of pancreatic ductal adenocarcinoma and 1 case of fibrosarcoma. The incidence of pancreatic cancer in group B within 3-5 months was 33.3% ( 12/36 ) , including 11 cases of pancreatic duetal adenocarcinoma and 1 case of fibrosareoma. The maximal diameter of mass was larger in group A than in group B ( P 〈 0.05). No pathological changes were found in pancreas of group C and other main organs of all groups. (2) The positive expression rate of IGF-1 and IGF-1R in groups A and B was significiantly higher in ductal adenocarcinoma than in non-cancerous pancreatic tissues ( P 〈 0.01 ). The positive expression rate of IGF-1 and IGF-1R in group A was significantly higher in ductal adenocarcinoma than in non-cancerous pancreatic tissues ( P 〈0.05 ), but no statistically significant difference was found in group B ( P 〉0.05 ). The positive cases of IGF-1 and/or IGF-1R in non-cancerous pancreatic tissues showed mild-to-severe atypical hyperplasia of ductal epithelia. Panc
