26例贲门溃疡癌变的内镜随访分析

摘　　要：目的探讨贲门溃疡癌变的内镜随访结果。方法对2000年1月至2008年6月期间高州市人民医院内镜室检出的26例贲门溃疡患者逐一登记,详记内镜下表现,根据病理结果分为单纯溃疡组与溃疡伴异型增生组进行为期3～6个月的随访复查,观察内镜下改变与病理结果。结果贲门溃疡发病率低,好发于男性贲门小弯侧,总体愈合率30.8%(8/26),癌发率26.9%(7/26),两组之间愈合率及癌发率差异均无显著性。结论贲门溃疡有别于其他部位的胃溃疡,不易愈合,癌发率较高,不管是否合并异型增生,应积极治疗,缩短随访时间。Objective To explore the prognosis after short follow-up of cardial ulceration cases. Methods Follow up 26 cardial ulceration cases from gastroscope check-up room from Jan 2000 to Jun 2008, and register the behavior under gastroscope. According the pathology results, they were divided into simple ulceration group and ulceration with atypical heteroplasia. We observed and register the change of gastroscope and pathology after 3 to 6 months follow-up. Results Cardial ulceration was a low incidence rate, it was often dignosed at cardia and gastric lesser curvature in male. The total heal rate was 30.8%, the cancer incidence rate was 26.9%, no significant difference was found between two groups. Conclusion Cardial ulceration was a specific ulceration with a low curabilty and high canceration rate, we should shorten the follow up term and have active treatments.
