卵巢恶性上皮性肿瘤p53、c-erbB-2癌基因表达及其临床病理学意义

摘　　要：目的探讨卵巢恶性上皮性肿瘤中ｐ５３和ｃ┐ｅｒｂＢ┐２基因表达与肿瘤恶性程度和预后的关系。方法１０８例卵巢癌应用Ｓ┐Ｐ免疫组化法检测其ｐ５３和ｃ┐ｅｒｂＢ┐２基因产物，另有１０例卵巢良性上皮性肿瘤作为对照。结果在１０例卵巢良性上皮性肿瘤中，ｐ５３和ｃ┐ｅｒｂＢ┐２产物均为阴性，在３７例交界性肿瘤中，其阳性率分别为２９．７％和２４．３％，７１例卵巢癌则高达５０．７％和５３．５％。在卵巢癌的不同组织学类型中，粘液性癌ｐ５３产物的阳性率高于浆液性和内膜样癌。在肿瘤分化方面，低分化癌的阳性率明显高于高分化癌组。在随访３～１４年的５６例中，ｐ５３和ｃ┐ｅｒｂＢ┐２基因的表达分别为２６．１％和２８．６％，明显低于死亡组（６６．７％和７７．８％），其差别是很明显的。结论上述结果表明，ｐ５３和ｃ┐ｅｒｂＢ┐２基因在卵巢恶性上皮性肿瘤中高表达，则其生物学行为和预后均较差。Objective:To evaluate the relationship of p53 and cerbB2 gene expression of the ovarian carcinomas with the malignancy and prognosis.Methods:The p53 and cerbB2 genes of 108 cases of ovarian carcinoma were labeled with SP immunohistochemical method, another 10 cases of benign ovarian tumors used as control.Results:The p53 and cerbB2 genes did not expression in 10 benign ovarian tumors , in 37 borderline cases their positive rates were 29.7% and 24.3% respectively, as to the 71 cases of ovarian carcinomas which were as high as 50.7% and 52.5% respectively. In various histologic types of ovarian carcinomas, the positive rate of p53 of the mucinous carcinoma was higher than the serous and endometrioid carcinoma, and the poorly differentiated carcinoma group was higher than the well differentiated group. In 56 cases followed for 3～14 years, the expression of p53 and cerbB2 genes in the survival group were 26.1% and 28.6% respectively, they were much lower than the deceased group (66.7% and 77.8%), the differences were significant statistically.Conclusion:This indicated that the higher level of p53 and cerbB2 gene expressed in the malignant ovarian tumors revealed a bad biological characters and a poor prognosis.
