摘  要:目的评价不同频率丘脑底核(subthalamic nucleus,STN)电刺激对癫痫大鼠模型的抗癫痫效果。方法实验大鼠分为四组,记录电极置于双侧额叶及海马。在三组电刺激组中,我们使用了三种不同的频率,分别为30、130和260Hz。用立体定向方法将刺激电极置入至双侧丘脑底核。在对照组中,同样方法置入刺激电极,但不进行电刺激。电刺激30分钟后,腹腔注射海人酸诱导急性边缘叶发作,对脑电和发作强度进行评价。结果130和260Hz高频电刺激明显减少了脑电大发作的时间,其中以130Hz为著,但发作的总时间没有改变。130和260Hz高频电刺激延长了局部放电的时间,这可能是因为电刺激抑制了继发的大发作的结果。此外,这两种频率的刺激降低了发作的程度,延长了第一次发作的潜伏期,但是,30Hz频率的刺激无上述效果。结论基底节系统参与大发作的控制,表明高频电刺激可被用于某些发作形式的控制。Objective To evaluate the antiepileptic effects of deep brain stimulation (DBS) on the subthalamic nucleus(STN) in an experimental seizure model at different frequency. Methods Rats were divided into four groups. Recording electrodes were placed on bilateral frontal regions and hippocampus in all rats. In the three STN--DBS groups, we determined the effects of three stimulation frequencies: 30 Hz, 130Hz and 260 Hz. DBS electrodes were stereotactieally implanted into the bilateral STN. After 30 minutes of stimulation, acute limbic seizures were induced through the intraperitoneal injection of Kainic acid (KA). Changes on electroencephalography (EEG) and the magnitude of seizures were evaluated. Results Results showed that STN stimulation at 130 Hz and 260 Hz significantly reduced the duration of generalized seizures on EEG, especially at 130Hz , although the total duration of all seizures (generalized plus focal seizures) was unchanged. The duration of focal seizures on EEG was prolonged by STN DBS at those frequency, possibly due to the suppression of secondary generalization. In addition, these two stimulations reduced the severity of clinical seizures and delayed the latency of first seizure. However, stimulation at 30 Hz demonstrated no effect. Conclusion These results confirm the involvement of the basal ganglia system in the control of generalized seizures and suggest that high frequency stimulations could be used in the treatment of some forms of seizures.
