老年冠心病伴焦虑抑郁的心理干预研究

摘　　要：目的研究心理干预对老年冠心病伴焦虑抑郁的影响。方法将103例老年冠心病伴焦虑抑郁的住院患者随机分为2组,对照组予单纯药物治疗,心理干预组予心理治疗结合药物治疗。2组在治疗前和治疗后均进行SDS、SAS和SAQ评分,在出院时进行住院时间及临床疗效总体评估。结果治疗后,2组SAS、SDS评分均下降,心理干预组的SAS、SDS评分显著低于对照组(P均<0.05),心理干预组下降程度与对照组相比更为显著(P均<0.01);而SAQ评分与SAS、SDS评分出现相反的变化。心理干预组住院时间较对照组明显缩短,有显著性差异(P<0.01);心理干预组临床疗效总体评价优于对照组(2=10.904,P<0.05)。结论心理干预对老年冠心病伴焦虑抑郁有很好的治疗效果,可缩短住院时间。Objective It is to observe the influence of the psychological intervention on coronary heart disease in elderly with depression and anxiety. Methods 103 elderly patients with coronary heart disease and depression anxiety were randomly divided into two groups： the control group was treated by the simple drug treatment, and the psychological intervention group was treated by psychological intervention combined with drug treatment. Both the two groups were scored by SOS, SAS and SAQ before and after treatment. Hospitalization time and clinical efficacy were evaluated when the patients were discharged. Results After treatment, the scores of SAS and SDS of the two groups both decreased, but the psychological intervention group were significantly lower than the control group （P 〈 0.05）, and the average scores of the psychological intervention group was decreased more significantly （P 〈 0.01）; however, the SAQ scores showed reverse change with SAS and SDS. Hospitalization time of the psychological intervention group was shorten than that of the control group; the whole clinical efficacy of the psychological intervention was better than the control group （X^2 = 10. 904, P 〈 0. 05）. Conclusion Psychological intervention has a good therapeutic effect to coronary heart disease with depression and anxiety in elderly and can shortened the duration of hospitalization.
