十二指肠溃疡患者治疗前后血清GAS,MTL,EGF及CGRP水平的变化

摘　　要：目的：探讨消化性溃疡与胃肠激素的关系，进一步了解十二指肠溃疡（ＤＵ）的发病机理，为临床治疗提供依据。方法：采用放免法动态观察３０例ＤＵ者治疗前后血清胃泌素（ＧＡＳ）、胃动素（ＭＴＬ）、表皮生长因子（ＥＧＦ）及降钙素基因相关肽（ＣＧＲＰ）的变化，并设正常对照组。比较治疗前后这些激素的改变并进行相关分析。结果：治疗前，４种激素与对照组比较均无明显差别，对照组中ＧＡＳ、ＥＧＦ呈负相关；治疗前ＤＵ组ＧＡＳ与ＣＧＲＰ呈负相关；治疗后血清ＧＡＳ、ＥＧＦ水平较治疗前明显提高，而ＭＴＬ明显下降（Ｐ＜０．０５，Ｐ＜０．０１），溃疡不同时期及不同的ＧＡＳ水平病人血清中４种激素呈现不同变化。结论：消化性溃疡与许多胃肠激素关系密切，但不是单一的，在其发病及整个病程中这些激素的改变是不同步的。Objective:To explore relation between peptic ulcer(PU)and gut bormones,it may promote the understanding of pathogenetic mechanisms of PU Methods:The contents of serum gastrin(GAS),motilin(MTL),epidermal growth factor(EGF)and calcitionin gene related peptide(CGRP)in 30 PU patients before and afetr treatment were measured in series by RIA Eight healthy adults served as control group The level of these hormones was compared with each other before and after treatment and correlative analysis was carried out Besides,the comparison of hormones level in different stage of PU was perfomed Results:Before treatment there was no significant difference between serum level of 4 kinds of hormones in active duodenal ulcer patients and that in control group The negative correlations between GAS and EGF in control group and between GAS and EGF in patients with active DU were found Level of serum GAS and EGF increased markedly,but MTL decreased in DU patients afetr treatment(P<0 05),P<0 01,respectively) 4 kinds of hormones were varied in diferent stage and GAS level of PU patients Conclusion: There was closely relation between sone gut hormones and peptic ulcer,but not only one way Their change was not concordant
