前列腺癌DNA倍体和AgNORS计数及PCNA表达的检测

摘　　要：目的探讨前列腺癌细胞核ＤＮＡ倍体、核仁组成区嗜银蛋白（ＡｇＮＯＲＳ）计数及增殖细胞抗原（ＰＣＮＡ）阳性指数变化与癌分级及预后的关系。②方法应用计算机图像分析技术、银染色方法及免疫组化ＬＳＡＢ法，测定３０例前列腺癌病人ＤＮＡ倍体、ＡｇＮＯＲＳ计数和ＰＣＮＡ阳性指数的变化。③结果前列腺癌ＤＮＡ倍体、ＡｇＮＯＲＳ计数、ＰＣＮＡ阳性指数均明显高于前列腺增生症（ｔ＝７．２６７～７．５０２，Ｐ＜０．０１），并且随着前列腺癌细胞分化程度降低，逐渐增加（Ｈ＝４２．５３４，ｔ＝２．１５６～６．７５６，Ｐ＜０．０５，０．０１；Ｆ＝１９４．７７１，１０５．６０５，ｑ＝１３．２１４～１５．８１３，Ｐ＜０．０１）；２年内死亡者，其ＤＮＡ倍体、ＡｇＮＯＲＳ计数、ＰＣＮＡ阳性指数明显高于存活２年以上者（ｔ＝５．２５６～７．６０８，Ｐ＜０．００１）；前列腺癌病人ＤＮＡ倍体、ＡｇＮＯＲＳ计数、ＰＣＮＡ阳性指数间均存在正相关关系（ｒ＝０．９６０～０．９６６，Ｐ＜０．００１）。④结论前列腺癌ＤＮＡ倍体、ＡｇＮＯＲＳ计数、ＰＣＮＡ阳性指数与癌细胞分化程度有关。Objective To study DNA ploidy, AgNORS count and proliferating cell nuclear antigen (PCNA) labeling index of prostatic carcinoma (PCa) and determine the relationships between these biologic markers and tumor grade and prognosis of the carcinoma. Methods DNA ploidy, AgNORS count and PCNA labeling index were evaluated by computerassisted image analysis technique, silver staining and immunohistochemitry (LSAB) in 30 cases of prostatic carcinoma and 20 cases of benign prostatic hypertrophy (BPH). Results DNA ploidy, AgNORS count and PCNA labeling index were higher in PCa than those in BPH (t=7.267—7.502, P<0.01), and increased along with the decreasing of tumor histological differentiation(H=42.534, t=2.156—6.756, P<0.05, 0.01; F=194.771, 105.605,q=13.214—15.813, P<0.01); the DNA ploidy, AgNORS count and PCNA labeling index in patients who died of PCa within two years were higher than those who survived for two years (t=5.256—7.608, P<0.001); DNA ploidy, AgNORS count, PCNA Labeling index were positively correlated with each other in PCa (r=0.960—0.966, P<0.001). Conclusion DNA ploidy, AgNORS count, PCNA labeling index were correlated with grading of prostatic carcinoma and could be used to predict the prognosis of PCa.
