白星ICE技术在微切口白内障超声乳化术中的应用

摘  要:目的探讨白星增加控制与效率(increased control and efficiency,ICE)技术在双手微切口白内障超声乳化手术中的临床效果。方法将115例(115眼)白内障患者随机分为对照组(56眼)和试验组(59眼),分别采用白星及白星ICE能量释放模式行双手微切口超声乳化白内障摘出+人工晶状体植入术。记录2组术中所用的平均超声能量、有效超声乳化时间、术后视力、角膜水肿程度,并观察手术并发症的发生情况。结果对照组平均超声能量分别为:Ⅱ级核(9.23±1.11)%,Ⅲ级核(14.11±3.01)%,Ⅳ级核(19.23±2.20)%;试验组平均超声能量分别为:Ⅱ级核(7.21±1.50)%,Ⅲ级核(11.99±3.20)%,Ⅳ级核(16.01±2.11)%。对照组有效超声时间分别为:Ⅱ级核(6.95±1.19)s,Ⅲ级核(11.10±1.53)s,Ⅳ级核(16.23±2.25)s;试验组有效超声时间分别为:Ⅱ级核(5.21±2.05)s,Ⅲ级核(8.99±2.35)s,Ⅳ级核(12.45±2.78)s。试验组Ⅲ级以上核硬度的平均能量和有效超声乳化时间均低于对照组,差异均有统计学意义(P均<0.05)。2组患者术后视力均较术前有明显提高。术后1d,对照组角膜无水肿者42眼,1级角膜水肿者11眼,2级角膜水肿者3眼;试验组角膜无水肿者53眼,1级角膜水肿者5眼,2级角膜水肿者1眼,2组间差异有统计学意义(P<0.05)。结论白星ICE技术能量模式可有效降低白内障超声乳化摘出术的有效超声乳化时间及总超声能量,从而减轻对角膜内皮的损伤,提高手术疗效。Objective To evaluate the clinical outcomes of White Star increased control and efficiency(ICE) technique in bimanual micro-incision phacoemulsification. Methods A total of 115 cataract patients( 115 eyes) were randomly divided into control group ( 56 eyes) and experimental group (59 eyes), and treated bimanual micro-incision phacoemulsification and IOL implantation. White Star system was taken in control group, and White Star ICE system in experimental group, The average power (AP), effective phacoemulsification time( EPT), postoperative visual acuity, degree of corneal edema and complications were recorded in both two groups. Results At grade Ⅱ, Ⅲ, Ⅳ of nuclear hardness , the AP were (9.23 ±1.11)%,(14.11±3.01)% and (19.23 ± 2.20)% ,respectively,in control group,and (7. 21 ± 1.50)% , ( 11.99 ± 3.20)% and ( 16.01 ± 2.11 ) % , respectively, in experimental group; The EPT were (6.95 ± 1.19 ) seconds, ( 1 1.10± 1.53 ) seconds and ( 16.23 ± 2.25 ) seconds, respectively, in control group, and (5.21 ± 2.05 ) seconds, ( 8.99 ± 2.35 ) seconds and ( 12.45 ± 2.78 ) seconds, respectively,in experimental group. The AP and EPT at the levels higher than grade Ⅲ of nuclear hardness in experimental group were significantly higher than those of control group ( both P 〈 0.05 ). The postoperative visual acuity was significantly higher than that of preoperative in both two groups. At postoperative 1 day, there were 42 eyes without corneal edema, 11 eyes with corneal edema at 1 degree and 3 eyes with corneal edema at 2 degree in control group,while 53 eyes without corneal edema,5 eyes with corneal edema at 1 degree and 1 eye with corneal edema at 2 degree in experimental group. There was significant difference between two groups (P 〈 0.05 ). Conclusion The White Star ICE system can effectively decrease EFT and AP in phacoemulsification for lessening the corneal epithelium injury and improving curative effects.
