摘 要:目的探讨激光光凝治疗糖尿病视网膜病变(diabetic retinopathy,DR)的临床效果。方法回顾分析368例(651眼)DR患者的临床资料,将患者的治疗分期分为治疗期、治疗近期及治疗远期;对激光光凝的方式、治疗后的视力、并发症等进行分析。结果患者在治疗期、治疗近期及治疗远期视力不变者分别为144例(39.1%)、148例(40.2%)、150(40.8%);视力提高≥3行分别为38例(10.3%)、39例(10.6%)、30例(8.2%);视力提高≥1行分别为52例(14.1%)、76例(20.7%)、79例(21.5%);视力下降≥3行分别为64例(17.4%)、49例(13.3%)、58例(15.8%);视力下降≥1行分别为70例(19.0%)、56例(15.2%)、51例(13.9%)。治疗期及治疗近期并发症以疼痛及白内障加速形成等发生率最高,分别为133眼和71眼。远期以牵引性视网膜脱离最多为88眼。结论激光光凝治疗DR有效,提高视力有时限,远期多因并发症视力降低。Objective To investigate the clinical significance of laser coagulation for diabetic retinopathy. Methods The clinical data of 368 cases (551 eyes) with diabetic retinopathy were reviewed. The patients were divided into three groups: during treatment,at a short term after treatment and at a long term after treatment. The ways of laser coagulation, vision and complications after treatment were analyzed. Results During treatment,at a short term after treatment and at a long term after treatment,patients whose vision unchanged were 144 cases(39.1% ) ,148 cases(40.2% ) and 150 cases(40.8% ) ,respectively;Whose vision increased be and more than 3 lines were 38 cases( 10.3% ), 39 cases ( 10.5% ) and 30 cases ( 8.2% ), respectively; Whose vision increased be and more than 1 line were 52 cases( 14.1% ) ,76 cases(20.7% ) and 79 cases (21.5%) ,respectively;Whose vision decreased be and more than 3 lines were 64 cases (17.4%) ,49 cases ( 13. 3% ) and 58 cases ( 15. 8% ), respectively; whose vision decreased be and more than 1 line were 70 cases( 19.0% ) ,56 cases( 15.2% ) and 51 cases ( 13.9% ) ,respectively. Pain( 133 eyes) and cataract developed quicldy(71 eyes) were the highest complications during treatment and at a short term after treatment, and trac-tional detachment of retina(88 eyes) was the highest complication at a long term after treatment. Conclusions Laser coagulation is an effective way for diabetic retinopathy, but it has time limit for improving vision. The vision always decreases for complications at a long term after treatment.
