认知治疗与人际心理治疗对产后抑郁症的疗效比较

摘　　要：目的探讨认知治疗与团体人际心理治疗对产后抑郁症患者的疗效及差异。方法完成全程治疗及随访的产后押郁症患者共110例，37例予认知治疗，38例予团体人际心理治疗，35例接受一般随访。治疗前、治疗后3个月及6个月末分别对3组进行爱丁堡产后抑郁量表（EPDS）、汉密尔顿抑郁量表（HAMD）、功能失调性态度问卷（DAS）及人际问题问卷（IIP）测试。结果认知治疗组和人际心理治疗组随访3个月和6个月后的EPDS总分及HAMD总分均明显低于对照组（P〈0．05），认知治疗及人际心理治疗组治疗后3个月及6个月的EPDS、HAMD总分低于治疗前（P〈0．05）。认知治疗组治疗后6个月的EPDS总分、HAMD总分及DAS总分均明显低于人际心理治疗组（P〈0．05）。结论认知治疗和团体人际心理治疗对产后抑郁症均有疗效，但认知治疗疗效更为显著。Objective To evaluate and compare the efficacy of cognitive behavioral therapy and group interpersonal psychotherapy for the patients with postpartum depression （PPD）. Methods 110 patients diagnosed with PPD according to DSM-- IV diagnostic criteria were enrolled in this study. These patients were assigned randomly to one of three groups, cognitive behavioural therapy （CBT） group, group interpersonal psychotherapy （GIPT） group or routine primary care group （control group）. They were assessed by using EPDS--10, H AMD--17, DAS, IIP at pre--treatment and the end of the third and the sixth month after treatment. Results After three months and six months follow--up, the total score of EPDS and HAMD was significantly reduced in CBT and GIPT groups compared to control group （P 〈 0.05）, and that was also remarkably lower than pre-- treatment （P 〈 0.05）. The EPDS total score, HAMD total score and DAS total score were significantly lower in CBT group than that in GIPT group after the sixth month follow-- up （P 〈 0.05）. Conclusions Cognitive behavioral therapy and group interpersonal psychotherapy are both effective for post--parturn depression, especially the cognitive behavioral therapy.
