摘 要:目的研究急性内囊出血后大鼠空肠肠系膜微循环的变化,探讨脑出血并发症的发病机理。方法采用自体尾静脉血注入法建立大鼠急性内囊出血模型,将40只Wistar大鼠随机分为假手术组、内囊出血后3、6、12、24h组,每组8只大鼠。运用微循环显微摄像分析系统,计测各组肠系膜微静脉血流量、切变率、白微栓数,并对切变率和白微栓数进行相关性分析;计测微动脉内轴流和边流的宽度,并计算边流宽度与管径的比值。结果与假手术组比较,急性内囊出血各时间点肠系膜微静脉血流量明显减少(P<0.05),脑出血6、12、24h组切变率减小(P<0.05),而白微栓数增加(P<0.05),切变率与白微栓数呈明显负相关(r=-0.917,P<0.05);脑出血6、12、24h组微动脉内边流宽度与管径的比值较假手术组显著减小(P<0.05)。结论急性内囊出血后大鼠肠系膜微血管血流量减少,切变率降低,白微栓数增多,边流宽度与管径的比值减小,这些微循环障碍可进一步导致靶器官的损伤。Objective To observe the changes of jejunal mesenteric microcirculation before and after acute internal capsule hemorrhage in rats, and to investigate the pathogenesis of complication after cerebral hemorrhage. Methods Internal capsule hemorrhagic animal model is duplicated by injecting 100μl autologous venous blood from tail into the hind limb of internal capsule. Forty rats were randomly distributed into the sham operated group, 3 h, 6 h, 12 h and 24 h groups after internal capsule hemorrhage. A microcirculatory microscope image analysis system was used to observe the jejunal mesentery microcirculation in rats. Volume of blood flow, wall shear rate, microthrombus of veinules, width of marginal flow and axial flow in the arterioles were measured. Results Compared with the sham operated group, blood flow of veinules in all internal capsule hemorrhage groups decreased significantly( P 〈0.05). In 6 h, 12 h and 24 h groups, wall shear rate of venules decreased markedly compared with the sham operated group( P 〈 0. 05), while microthrombus increased( P 〈 0. 05). Wall shear rate was inversely correlated to microthrombus ( r = - 0. 917, P 〈 0.05 ) . The width of marginal flow in the cerebral hemorrhage rats decreased, the ratio of width of marginal flow to width of arteriole reduced significantly( P 〈 0. 05). Conclusion The significant microvascular abnormality and blood - flow dysfunction is shown in mesenteric microcirculation of acute internal capsule hemorrhage rats, which can damage the target organs.
