摘  要:目的探讨乳腺癌及其切缘组织中hTERT活性表达、存在的分子边界及其与手术切缘安全性的关系。方法用SABC免疫组化法检测20例乳腺癌标本及其周边上、下、内、外象限距癌肿边缘1、2、3cm切缘组织中的hTERT活性表达。结果20例患者癌组织和癌周不同距离(1、2、3cm)的组织内,hTERT阳性表达在癌周3、2、1cm的组织到癌组织之间呈现出逐渐增强的趋势(P〈0.001),各象限相同距离的hTERT阳性表达率差异不具统计学意义(P〉0.05)。结论(1)hTERT在乳腺癌肿瘤组织中高表达,乳腺癌在四周象限的切缘组织hTERT高表达范围均在3cm以内;(2)hTERT所表现出的趋势性差异表达提示在乳腺癌周围组织中存在着分子边界,距离肿块3cm可定为乳腺癌安全切除的分子边界。Objective To detection of the expression of human telomerase reverse transcriptase (hTERT) in breast cancer and adjacent tissues in relation with the tumor surgical margins. Methods The expressions of hTERT was detected with SABC immunohistochemistry in 20 patients with breast cancer, the examined samples included tumor tissue, and adjacent normal tissues 1, 2 and 3 cm from the tumor margin in 4 quadrants around the breast. Results The expression of hTERT in the cancer tissue was the highest and the intensity of expression was decreased with the distance from the tumor margin (P〈0.001). There were no significant differences of hTERT expression in same distance of all quadrants round the breast (P〉0.05). Conclusion The results indicate that hTERT might be expressed in adjacent tissues within 3cm from the tumor margin, so that 3 cm can be defined as the safe molecular margin of lumpectomy for patients with breast cancer.
