摘 要:目的通过应用SELDI—TOF—MS蛋白质芯片技术筛选前列腺癌患者不同于前列腺增生患者和正常健康男性的血清标志蛋白,探讨诊断前列腺癌的新方法。方法检测79例研究对象(包括前列腺癌22例、前列腺增生26例、正常健康男性31例)血清蛋白质图谱,并采用Proteinchip 3.0软件进行分析,选出最佳前列腺癌诊断模型,应用该模型对同期另择的12例前列腺癌患者、18例前列腺增生患者以及18例体检健康者进行盲法验证。结果(1)与正常健康男性比较,前列腺癌患者血清中5个标志蛋白呈高表达(分子量分别为11665、2958、7978、3092及4657Da),据此建立诊断模型,灵敏度为95.45%,特异性为83.87%;阳性预测值为80.77%,阴性预测值为96.30%。(2)与前列腺增生患者比较,前列腺癌患者血清中2个标志蛋白呈高表达、1个呈低表达(分子量分别为3191、3271及1127Da),据此建立诊断模型,灵敏度为95.45%,特异性为96.15%;阳性预测值为95.45%,阴性预测值为96.15%。所有盲法验证结果均较满意。结论SELDI—TOF—MS蛋白质芯片技术可以筛选出前列腺癌标志蛋白并建立前列腺癌诊断模型,可能成为前列腺癌诊断的有效方法。Objective To identify the serum protein fingerprinting as the biomarkers of prostate cancer. Methods Serum samples were collected from 22 cases of prostate cancer (PCA), 26 benign prostate hyperplasia( BPH )and 31 healthy subjects. The serum protein fingerprinting was detected by surface-enhanced laser desorption/ionization mass spectrometry (SELDI- MS). The data of spectra were analyzed by Proteinchip 3.0. Results Compared with the spectra of healthy men, there was high expression of 5 proteins (11665Da, 2958 Da, 7978 Da, 3092 Da and 4657 Da) in PCA patients. The software Biomarkerwizard and Biomarker Pattern automatically, under given conditions, were used to establish a decision tree to differentiate the PCA from healthy subjects with a specificity of 95.45% and a sensitivity of 83.87%. Compared with the spectra of BPH there was high expression of proteins 3191 Da and 3271 Da, and low expression of protein 1127 Da in PCA patients, which were used to differentiate PCA from BPH with a specificity of 95.45% and a sensitivity of 96.15%. Conclusion SELDI mass spectrometry coupled with decision tree classification algorithm can identify new serum biomarkers, which will provide an innovative approach for the diagnosis of PCA.
