原发性胃癌REG Ⅰα蛋白表达与临床病理特性的相关性研究

摘　　要：目的探讨原发性胃癌组织中REG Ⅰα蛋白的表达与胃癌临床、病理特性和患者生存率之间的关系。方法采用免疫组化法检测235例原发性胃癌患者组织中REG Ⅰα蛋白的表达，并分析其与胃癌临床、病理特征的相关性。结果73例患者REG Ⅰα（蛋白表达呈阳性31.1％），其中呈浸润性生长者REG Ⅰα蛋白阳性率高于呈局限性生长者（P〈0．05）；印戒细胞癌者胞质呈REG Ⅰα蛋白强表达，且阳性率明显高于非印戒细胞癌者（P〈0．05）；REG Ⅰα蛋白呈阳性的高分化腺癌患者出现淋巴结和血行转移率均高于REG Ⅰα蛋白阴性者（均P〈0．05或001）；REG Ⅰα蛋白表达呈阴性的高分化腺癌患者5年生存率显著高于表达呈阳性者（P〈0．01）。结论REG Ⅰα蛋白的表达与胃癌细胞的分化状态和细胞浸润性生长有关，并可能为评估高分化胃腺癌预后的重要指标。Objective To investigate the relation between expression of REG Ⅰα protein, its clinicopathologic parameters in patients with primary gastric cancer. Methods Using immunohistochemical techniques, the expression of REG Ⅰα protein was investigated in 235 tissue samples from patients with gastric carcinoma. Results Seventy three out of 235 patients showed a positive REG Ⅰα protein （31.1%）. REG Ⅰαprotein expression in infiltrating tumors was significantly higher than that in localized tumors （P〈0.05）. REG Ⅰα expression in the cytoplasm of signet ring cell carcinoma was significantly stronger than in other types of tumor. The patients with REGⅠα-positive differentiated adenocarcinoma had a significantly higher rate in lymphatic metastasis and venous invasion compared with REGⅠα - negative tumor. The 5-year survival rate in the patients with REG Ⅰα-negative differentiated adenocarcinoma was significantly higher than those with REGⅠα-positive tumors. Conclusion Results demonstrate that the expression of the REG Ⅰα protein is closely related to the infiltrating property of primary gastric carcinoma, and may be used as a prognostic indicator for differentiated adenocarcinoma of the stomach.
