闭角型青光眼高眼压下小梁切除术临床观察

摘　　要：目的观察闭角型青光眼高眼压状态下进行小梁切除术的临床疗效。方法56例(56眼)持续高眼压患者在药物治疗无效的情况下进行小梁切除术,观察手术前后视力、眼压、前房以及手术并发症。结果手术顺利,没有发生严重的并发症。手术前后视力比较z=-9.038,P=0.000,经统计学分析有统计学意义,治疗后视力高于治疗前;手术前后眼压比较t=22.830,P=0.000,经统计学分析有统计学意义,治疗后眼压明显下降。结论闭角型青光眼高眼压状态下及时进行小梁切除术是安全的,能挽救患者的视功能,但是在手术过程中应注意手术技巧,避免并发症的发生。Objective To observe the efficacy of the trabecutectomy for angle closure glaucoma with high intraocular pressure （lOP） . Methods Total 56 cases （56eyes） of angle - closure glaucoma with high intraocular pressure （IOP） underwent trabeculectomy. The IOP , visual acuity and complications etc were observed. Results No severe complications such as choroidal hemorrhage and malignant glaucoma were observed in all cases during surgery and postoperatively. Among all of 56 eyes , the postoperative visual acuity improved and there were significant difference compared with the preoperational visual acuity （z = -9. 038, P -0. 000） ; the postoperative IOP descended and there were significant difference compared with the preoperational IOP （ t = 22. 830, P = 0. 000）. Conclusion Trabeculectomy for angle - closure glaucoma with persistent high intraocular pressure is safe and effective. It can keep the visual function from more damage but we should pay attention to the skill of the operation.
