MCM2免疫细胞化学检测在宫颈癌筛查中的意义

摘　　要：目的:探讨MCM2在宫颈癌和癌前病变中的表达及其临床意义。方法:选择宫颈液基细胞学剩余标本进行MCM2免疫细胞化学标记,MCM2标记结果与细胞学结果和活检组织学结果对照分析。结果:共收集了187例宫颈液基细胞学剩余标本,其中172例有活检组织学结果。MCM2在细胞学诊断的SCC、HSIL、LSIL、ASC-H和ASC-US中阳性表达率分别为100.0%、97.1%、68.1%、100.0%和54.6%,在活检组织学确诊的SCC、CIN3、CIN2和CIN1中阳性表达率分别为100.0%、92.2%、75.0%和85.7%。在40例非典型鳞状上皮细胞(ASC)中,MCM2检测对筛选组织学确诊的CIN1及以上病变敏感性和特异性分别为85.7%和80.0%;对筛选CIN2及以上病变的敏感性和特异性分别为77.8%和86.7%。结论:MCM2在宫颈癌和癌前病变中高表达,并可作为宫颈癌筛查和ASC管理的辅助性标志。OBJECTIVE：To explore the expression of MCM2 in cervical carcinoma and precancerous lesions and evaluate it utility. METHODS： MCM2 was detected as a marker of aberrant S-phase induction in cervical cytology specimens by an immunocytochemistry method, and results were compared with cytology diagnosis and histologic biopsy diagnosis. RESULTS： A total of 187 residual cytology specimens were collected. Among them 172 cases were available for a histologic biopsy diagnosis. According to cytology categories, MCM2 positive staining were detected in 100. 0% of the SCC, 97.1% of the HSIL, 68. 1% of the LSIL, 100. 0% of the ASC-H and 54.6% of the ASC-US. Among cases with biopsy diagnosis, the percentages of positive MCM2 were 100.0% of the SCC, 92.2% of the CIN3, 75. 0% of the CIN2 and 85. 7% of the CIN1. Using MCM2 positive staining as a threshold, the MCM2 assay yielded a sensitivity of 85.7%, and specificity of 80.0% for detecting CIN1 or worse in patients with an ASC diagnosis. Whereas the sensitivity was 77.8% and the specificity was 86.7% for detecting CIN2 or worse. CONCLUSION： MCM2 is overexpression in cervical carcinoma and precancerous lesions, and MCM2 may serve as a useful marker in the screening of cervical carcinoma and may help in further upgrading the diagnosis of ASC.
