急性髓细胞白血病CDX2基因表达及临床意义的探讨

摘　　要：目的:探讨CDX2基因在急性髓细胞白血病(AML)患者的表达及临床意义。方法:初发AML患者66例,采用RT-PCR方法检测CDX2基因的表达,观察该基因表达与初治患者治疗效果的关系。结果:66例AML患者中,CDX2基因阳性表达56例,阳性率84.8%。51例非M3型AML患者中,CDX2表达阳性与阴性的完全缓解(CR)率分别为81.8%和71.4%,两者比较差异无统计学意义,P=0.896。对表达阳性的13例AML患者治疗后CDX2基因表达行动态观察,当获得CR时,CDX2表达可转阴性,复发时转为阳性,持续阳性者可早期复发;未获CR者可持续表达阳性。结论:CDX2基因在AML患者中有较高的表达。CDX2基因表达是否阳性对非M3型AML治疗CR率无影响。监测CDX2基因表达变化可作为检测微小残留病的有效手段。OBJECTIVE： To study the expression of CDX2 in acute myelogenous leukemia （AML）and its clinical significance. METHODS： Sixty-six newly diagnosed patients with AML were chosed. The expression of CDX2 mRNA was detected by RT-PCR. The relationship between CDX2 expression and the respone to treatment was analyzed. RESULTS： The expression of CDX2 was positive in 56 of 66 AML patients （84. 8%）. The CR rate was 81.8% in CDX2-positive non-M3 AML patients and 71. 4% in CDX2-negative non-M3 AML patients, which showed no difference （P=0. 896）. Thirteen patients with positive CDX2 expression were followed up. The CDX2 could turn negative after got CR and turn positive again when relapsed. The patients who had gotten CR could relapse earlier when the CDX2 was continent positive. The CDX2 positive expression eouldnot turn negative when the patients did not get CR. CONCLUSIONS： CDX2 expression is observed in most AML patients. Whether the CDX2 expression is positive or not does not effect on the CR rate in AML. Monitoring of the CDX2 expression may be a useful means for the detection of minimal residual disease in AML.
