摘 要:目的:探讨CD15抗原、CD44V6蛋白及增殖细胞核抗原(PC-NA)在乳腺癌鉴别诊断中的价值。方法:采用SP法检测CD15抗原、CD44V6蛋白及PCNA在乳腺癌、乳腺增生症及副乳组织中的表达。结果:CD15抗原在51例乳腺癌组织、67例乳腺增生组织中及10例正常乳腺组织中的阳性表达率分别为78.4%、44.8%和20.0%,CD44V6蛋白的阳性表达率分别为80.4%、37.3%和20.0%,PCNA的阳性表达率分别为88.2%、68.7%和30.0%,这3个指标表达在乳腺癌与乳腺增生之间差异有统计学意义,P<0.05;CD15抗原、CD44V6蛋白及PC-NA阳性表达率与乳腺癌患者腋淋巴结状况及组织学分级有显著相关性(P<0.05),而与患者年龄、绝经状况、TNM分期无显著相关性,腋淋巴结转移阳性患者阳性表达率(91.9%、86.5%和97.3%)明显高于腋淋巴结转移阴性者(42.9%、64.3%和64.3%),组织学分级Ⅱ+Ⅲ级阳性表达率(88.9%、91.7%和94.4%)明显高于Ⅰ级者(53.3%、53.3%和78.6%)。乳腺癌组织CD15、CD44V6和PCNA表达均呈正相关性,P均<0.001。结论:联合检测CD15抗原和CD44V6蛋白及PCNA可能对乳腺良恶性病变的鉴别诊断具有重要的参考价值,三者对判断乳腺癌的恶性程度、预测生物学行为和预后有参考价值。OBJECTIVE: To evaluate the clinical significance of CD15 antigen,CD44V6 protein and PCNA in differential diagnosis and early diagnosis of breast carcinoma. METHODS: The expressions of CD15 antigen and CD44V6 protein in 51 cases of breast carcinoma, 67 cases of breast hyperplasia and 10 cases of accessory mammary gland tissues (replacement for normal breast) were investigated by SP immunohistochemical technique. RESULTS: The positive expressive rates of CD15 antigen in 51 cases of breast carcinoma, 67 cases of breast hyperplasia and 10 cases of normal breast tissues were 78.4%, 44. 8% and 20.0% respectively; the positive expressive rates of CD44V6 protein were 80. 4%, 37. 3% and 20.0% respectively; the positive expressive rates of PCNA were 88.2%, 68.7% and 30.0% respectively. There was a significant difference between breast carcinoma and breast hyperplasia(P〈0.05). The positive expressive rates of CD15 antigen, CD44V6 protein and PCNA were 91.9%, 86.5% and 97.3% respectively for patients with axillary lymph node metastasis, significantly higher than those without metastasis (42.9%, 64.3%, 64.3% respectively). The positive expressive rates of CD15 antigen, CD44V6 protein and PCNA were 88.9%, 91.7% and 94.4% respectively for patients of histological grading Ⅱ +Ⅲ, significantly higher than those of histological grading Ⅰ(53.3%, 53.3% and 78.6% respectively). Moreover, there was a positive correlation between CD15 and CD44V6 expressions in breast carcinoma(r=0.43, P〈0. 001);positive correlation between CD15 and PCNA expressions in breast carcinoma (r= 0.48, P 〈 0. 001) ; positive correlation between CD44V6 and PCNA expressions in breast carcinoma(r=0.34, P〈 0.001). CONCLUSIONS The expressions of CD15 antigen, CD44V6 protein and PCNA may be related to the carcinogenesis and progression of breast carcinoma. Combined detection of CD15 antigen, CD44V6 protein and PCNA may be useful in differential diagnosis between benign and malignant lesions of breast.
