Nd:YAP激光联合平阳霉素治疗喉及喉咽血管瘤的临床分析

摘　　要：目的探讨Nd:YAP激光联合平阳霉素治疗喉及喉咽部血管瘤的临床疗效。方法在全麻并支撑喉镜显微镜下用功率3～5WNd:YAP激光以非接触式扫描照射喉咽血管瘤至组织苍白萎缩,再按肿瘤大小,分别以不同量的平阳霉素分点均匀注入残留肿瘤组织内至肿瘤组织稍肿胀。结果本组11例喉及喉咽血管瘤病患者1次治疗治愈3例,2次治疗治愈3例,3次治疗治愈2例,4次治疗基本治愈1例,总有效率81.8%;经4次治疗好转2例。平均治疗次数2.5次。术后瘢痕形成3例,伤口感染1例。结论Nd:YAP激光联合平阳霉素治疗喉及喉咽血管瘤,手术操作精确、简便、安全,术中无出血,术后患者反应轻,恢复快,治愈率高,并发症发生率低。Objective To explore the clinical effect of Nd ： YAP laser combined with bleomycin （pingyangmycin） on treating larynx and laryngo-pharynx hemangioma. Methods By general anesthesia and laryngeal endoscope, using 3-5W of Nd： YAP laser to irradiate larynx and laryngopharynx hemangioma by the way of non-contact scanning until the tissues changed pale and shrank. Then based on the differed size of the tumors, different dosages of bleomycin were evenly injected into the remaining tumor tissues until the tissues were a little swelling. Results Of the 11 cases in the present study, 3 cases were cured after once of therapy, 3 cases were cured after twice of therapy, 2 cases were cured after 3 times of therapy, 1 case was basically cured after 4 times of therapy; the total effective rate was 81.8% and 2 cases improved after 4 times of therapy. The average therapy times were 2. 5. The incidence of the main complications such as scarring was in 3 cases and wound infection was in 1 case.Conclusions Nd ： YAP laser combined with bleomycin is used to treat larynx and laryngopharynx hemangioma with precise, easy, convenient and safe operation, as well as non-hemorrhage. The patients have slight post-operation reaction, quick recovery, high healing rate and low incidence of complications, which contribute to a treatment method deserving spread extensively.
