泪道YAG激光成形联合鼻泪管逆行植入球头硅胶管治疗鼻泪管阻塞

摘　　要：目的探讨单纯泪道YAG激光成形术与泪道YAG激光成形联合鼻泪管逆行植入球头硅胶管手术在治疗鼻泪管阻塞方面的不同疗效,为基层临床眼科筛选一种安全简便、疗效可靠、低创伤的治疗方法。方法选择2005年1月至2007年6月就诊的鼻泪管阻塞患者125例152眼,将其随机分为A、B两组,A组采用单纯泪道YAG激光成形术,B组采用泪道YAG激光成形联合鼻泪管逆行植入球头硅胶管手术治疗。两组均于术后前3 d,每天以氧氟沙星注射液及地塞米松行泪道冲洗1次,以后每周冲洗1次至术后1个月。按疗效标准对两组治疗效果进行分析、比较。结果A组60例75眼,1个月时总有效率96%,3个月时总有效率85.33%,6个月时总有效率80.56%,第6个月时失访3眼。B组65例77眼,1个月时总有效率100%,3个月时总有效率96.1%,6个月时总有效率93.15%,第6个月时失访4眼。结论泪道激光成形联合鼻泪管逆行植入球头硅胶管较单纯泪道激光成形术疗效较为明显,尤其是远期疗效。采用泪道YAG激光成形联合鼻泪管逆行植入球头硅胶管方法,充分利用了激光治疗及球头硅胶管植入的各自优势,进一步降低了疾病复发的可能性,达到了双重的功效,值得基层临床广泛推广开展。Objective To investigate different curative effects of using simply YAG laser formation or it associating with reverse imbeding silicone tube of spherical cephalon. Thus, to obtain a kind of way which was safe, convenient, reliable curative effect and low-trauma for clinical ophthalmology of basement. Methods We chose 152 eyes of 125 patients in hospital for treatment of ophthalmic nasolacrimal duct obstruction from January 2005 to June 2007. The were randomly divided into A, B groups. Treated with lacrimal YAG laser angioplasty in A group, while we employed lacrimal passages YAG laser formation associating with reverse imbeding silicone tube of spherical cephalon in B group. After the treatments we enforced syphilis of lacrimal passages with ofloxacin and Dexamethasone injection once a day in the first 3 days and then once a week in a month. Eventually, we analyzed and compared effects of both groups according the standard. Results Total effective rate 96% in the 1st mouth , 85.33% in the 3rd mouth and 80. 56 % in the 6th month in A group, which included 75 eyes of 60 patients. Unfortunately, we lost 3 cases in the 6th month. While total effeetive rate was100% in the 1st month, was 96.1% in the 3rd month and was 93.15% in the 6th month in B group, which included 77 eyes of 65 patients, and we lost 4 cases in the 6th month. Conclusion The curative effect of using lacrimal passages YAG laser formation assoeiating with reverse imbeding silicone tube of spherical eephalon is more obvious comparing with using singly lacrimal YAG laser angioplasty, especilly in the prosteedtive efficacy. The former made full use of both advantages of the laser treatment and silicone tube of spherical eephalon implantation, reduced the possibility of recurrence and reached a double effectiveness. In short, it is valuable for elinieal ophthalmology of basement.
