综合治疗对稳定期精神分裂症患者认知功能的影响

摘　　要：目的比较单纯治疗与综合治疗对于稳定期精神分裂症认知功能的影响。方法将137名稳定期精神分裂症患者分为单纯治疗组70例，综合治疗组67例。用威斯康星卡片分类测验（WCST）、连线测验（TMTA和TMTB）、中国修订韦氏记忆量表（WMS—RC）的视觉再生和中国修订韦氏成人智力量表（WAIS．RC）的数字符号、数字广度，评定稳定期精神分裂症患者基线和随访12月末的认知功能，比较治疗前后患者认知功能的变化。结果单纯治疗组12月末WCST总正确数、完成分类数、总错误数均较基线显著改善（P〈0．01～0．05），TMTA和TMTB完成任务时间和笔离开纸次数均低于基线（P〈0．01），数字符号、数字广度、视觉再生成绩均显著优于基线（P〈0．01）；综合治疗组12月末数字符号、数字广度、视觉再生、TMTA完成任务时间和笔离开纸次数，以及WCST、TMTB的各项成绩均显著优于基线（P〈0．01）；综合治疗组12月末WCST各项成绩、TMTA完成任务时间（28．30±10．79）s、TMTA连接正确数（24．97±0．17）个、TMTA连接错误数（0．03±0．17）个、TMTB完成任务时间（56．19±27．15）s、TMTB笔离开纸次数（0．16±0．48）次、数字符号（65．58±7．91）分、数字广度（16．72±1．96）分及视觉再生（10．96±1．96）分成绩均优于单纯治疗组（P〈0．01～0．05）。结论非典型抗精神病药对稳定期精神分裂症患者的认知功能有改善作用，综合治疗对于认知功能改善更明显。Objective To compare the effect of simple antipsychoties therapy and integrated treatment on cognitive function of stable schizophrenia patients. Methods 137 stable schizophrenia patients were divided into two groups,70 patients in control group which using atypical antipsychoties only（ n = 70） and integrated treatment group which atypical antipsychotics combined with bio-psycho-social intervention （ n = 67 ）. Using Wisconsion Card Sorting Test （WCST） ,Trail Making Test （TMTA and TMTB） ,Digit Symbol and Digit Span in Wechsler Aduh In- telligence Scale Revised in China （WAIS-RC） ,Visual Reproduction in Wechsler Memory Scale-Revised in China （WMS-RC） to evaluate cognitive function of the Stable schizophrenia patients at the baseline and 12 months later. Results After 12 months,the results of control group such as total correct number of WCST, sorting of WCST,dig- it symbol and total wrong number of WCST were better than baseline（P 〈 0.01 -0.05 ） , the score of digit span and visual reproduction were higher than baseline,and the time finished TMTA and TMTB were less than baseline（P 〈 0.01 ）. After 12 months, the results of integrated treatment group such as digit symbol, digit span and visual repro- duction,WCST and TMT were better than baseline （P 〈 0. O1 ）. After 12 months, the results of integrated treatment group such as digit symbol（65.58 ± 7.91 ） , digit span（ 16.72 ±1.96 ） and visual reproduction（ 10.96 ± 1.96 ） , WCST ,TMTA and TMTB were better than control group （P 〈 0.01 -0.05 ）. Conclusion Integrated treatment were better on the cognitive function of stable schizophrenia patients than simple atypical antipsychotiea treatment.
