阅读疗法对缓解期精神分裂症患者自知力治疗依从性及复发的影响

摘　　要：目的探讨阅读疗法对缓解期精神分裂症患者自知力恢复、治疗依从性及复发的影响。方法将120例精神分裂症患眷随机分为研究组和对照组各60例。疗程8周，出院后随访1年。采用简明精神病量表（BPRS）、自知力与治疗态度问卷（ITAQ）、治疗依从性、住院天数及复发率评估患者的康复情况。结果治疗8周后：BPRS量表评分明显低于对照组[（21．55±8．56）分，（24．64±9．45）分]；ITAQ评分明显高于对照组[（10．10±4．81）分，（8．49±5．12）分]；治疗依从性研究组分别为：完全依从为75，0％，部分依从为20．O％，不依从为5．0％；对照组分别为51．7％、34．5％、13．8％，差异有显著性意义（P〈0．01）。1年末随访：BPRS量表评分明显低于对照组；ITAQ评分明显高于对照组；治疗依从性研究组分别为：完全依从为63．3％，部分依从为26．7％，不依从为10．0％；对照组分别为36．2％、25．9％、37．9％，差异有显著1生（p〈0．01）。复发率明显低于对照组，差异有显著性（P〈0．01）；平均住院天数明显短于对照组，分别为（58±7）d和（76±18）d，差异有显著性（P〈0．01）。结论阅读疗法不仅有助于精神分裂症患者精神症状的改善、而且能恢复自知力、缩短住院时间、提高治疗依从性和降低复发率。Objective To investigate the effect of the bibliotherapy on insight recovering, drug therapy compliance and recurrence in remitted schizophrenia patients. Method 120 schizophrenia patients were random- ly divided into the study group and the control group, with 60 cases in each group. The control group were only trea- ted with drugs and the study group were treated with drugs plus bibliotherapy. Compared after 8 weeks treatment and a one-year follow-up was carried out after their discharge from hospital. The clincal efficacy was evaluated by the Brief Psychiatric Rating Scale （BPRS） ,the Insight and Treatment Attitude Questionnaire （ITAQ）, drug thera- py compliance,hospital day and recurrence. Results After treatment 8 weeks,the scale of BPRS of study group lower than that of control group（ 21.55 ± 8.56 and 24.64 ± 9.45） , and the scale of ITAQ of study group higher than that of control group （ 10. 10 ±4.81 and 8.49 ± 5. 12） ; compliance of study group was better than that of control group （ totally compliance ：75% and 5 i. 7% , partly compliance ： 20.0% and 34.5% , none compliance ： 5.0% and 13.8% , P 〈 0.01 ）. At the end of 1 year,the scale of BPRS of study group lower than that of control group ;the scale of ITAQ of study group higher than that of control group and compliance of study group was better than that of control group （ totally compliance： 63.3% and 36.2% ,partly compliance ： 26.7% and 25.9% ,none compliance： 10.0% and 37.9% , P 〈 O. 01 ）. The recurrence rate of study group was lower than that of control group. The average hospital day of study group was shorter than that of control group [ （58 ± 7 ）d and （76 -± 18 ）d, P 〈 0.01 ]. Conclusion Bibliotherapy not only is beneficial to the treatment for schizophrenia patients, but also can makes shorter hospital day, lower of these patients the rate of recurrence.
