摘　　要：目的探讨应用自体真皮脂肪瓣游离移植矫治半侧颜面萎缩的临床价值。方法自体真皮脂肪瓣均取自下腹及髂腰部，切取范围为5cm×11cm～7cm×20cm。切取后游离移植，并配合Medpor假体植入、健侧下颌角外板截除移植于患侧、皮肤软组织扩张术、自体脂肪颗粒游离移植等方法．自2004年5月至2008年5月，对15例半侧颜面萎缩患者进行综合矫治。供区直接缝合。结果术后近期除1例患者出现部分脂肪液化经换药治疗愈合外，余者伤口均Ⅰ期愈合；随访15例患者6个月至4年，面部外形基本对称，效果较满意。结论半侧颜面萎缩应根据病情采用适当的方法进行矫治。自体真皮脂肪瓣具有供区组织量充足、成活率较高、供区隐蔽、切取操作简单等优点，值得临床上推广应用。Objective To investigate the effect of free dermal-fat flap in correcting the hemifacial atrophy. Methods The dermal fat flap from inferior belly and ilio - waist partes with the size ranged from 5 cm × 11 cm to 7 cm × 20 cm was free grafted with the Medpor prosthesis and the cortical plate taken from angle of mandible on the uninjured side to the injured side. These methods of skin and soft tissue expansion and autoallergic lipochondria transplantation were also adopted. From May 2004 to May 2008, 15 cases were corrected with dermal fat flaps and other additional methods according to the symptoms of the deformities. Donor sites were sutured directly. Results All patients were healed primarily except 1 case with fat liquefaction which was recovered by chang dressing. After 6 months - 4 years following up, 15 cases were satisfied with symmetrical facical contour. Conclusion It is a reasonable method for correcting the hemifacial atrophy with adequate tissue, simple operation, the concealed donor scar and higher suvival rate by combined ways according to the symptoms of the deformities. It is worth spreading.
