慢性牙周炎病人外周血CD4^+CD25^+调节性T细胞的检测和免疫抑制功能分析

摘　　要：目的:探讨慢性牙周炎病人外周血中CD4+CD25+调节性T细胞与牙周炎病变程度的相关性,并进一步分析CD4+CD25+调节性T细胞的免疫抑制功能。方法:应用流式细胞仪检测牙龈炎、慢性牙周炎病人和健康对照者外周血中CD4+CD25+调节性T细胞占CD4+T细胞的比例;以体外淋巴细胞混合培养法检测CD4+CD25+调节性T细胞对同源CD4+CD25-T细胞增殖的抑制效应,ELISA法检测培养体系中转化生长因子-β、白细胞介素-10的分泌水平。结果:轻度和中度慢性牙周炎组外周血中CD4+CD25+调节性T细胞占CD4+T细胞的比例分别为(19.33±7.04)%和(18.56±5.79)%,显著高于健康对照组和牙龈炎组(P<0.05);重度慢性牙周炎组为(9.31±3.04)%,显著低于健康对照组、牙龈炎组、轻度和中度慢性牙周炎组(P<0.05)。当与CD4+CD25-T细胞1∶1混合培养时,重度慢性牙周炎者CD4+CD25+调节性T细胞的抑制率较健康对照组、牙龈炎组、轻度和中度慢性牙周炎组降低(P<0.05),各培养组之间转化生长因子-β、白细胞介素-10的分泌水平无显著差异。结论:轻度和中度慢性牙周炎病人外周血CD4+CD25+调节性T细胞的表达上调,而重度慢性牙周炎病人则显著下降;重度慢性牙周炎病人CD4+CD25+调节性T细胞的抑制功能存在异常,可能与其接触抑制途径缺陷有关。AIM： To investigate the relationship between the frequency of CD4^＋ CD25^＋ regulatory T cells in the peripheral blood of patients with chronic periodontitis and the severity of periodontal destruction. And the suppressive function of CD4^＋ CD25^＋ regulatory T cells was further analyzed. METHODS ： Flow cytometric analysis was used to detect the frequency of CD4^＋ CD25^＋ regulatory T cells in the peripheral blood of patients with gingivitis or chronic periodontitis, as well as healthy controls. Furthermore, the direct suppressive effect of CD4^＋ CD25^＋ regulatory T cells on CD4^＋ CD25^- T lymphocyte proliferation was performed by the mixed lymphocytes reaction and measured by 3H - Thymidine radioactive assay. The secretion of TGF - β and IL - 10 in the cultures was measured by ELISA. RESULTS ： Patients with mild or moderate chronic periodontitis had a higher frequency of CD4^＋ CD25^＋ regulatory T cells in the peripheral blood[ （ 19.33% ±7.04% and 18.56%±5.79% respectively） ] than healthy controls and patients with gingivitis （P 〈 0.05 ）. The frequency in patients with severe chronic periodontitis （9.31 ± 3.04 ）% was decreased more than that in healthy controls and patients with gingivitis, mild or moderate chronic periodontitis （P 〈 0.05 ）. A significant decrease was found in the suppressive function of CD4^＋ CD25^＋ regulatory T cells from peripheral blood of patients with severe chronic periodontitis when co - cultured with CD4^＋ CD25^- T lymphocytes in the proportion of 1 ： 1 （P 〈0.05）. No significant differences were found in TGF - β and IL - 10 secretion between different culture groups. CONCLUSION ： The expression of CD4^＋ CD25^＋ regulatory T cells is up - regulated in patients with mild or moderate chronic periodontitis, however, is decreased in patients with severe chronic periodontitis. And the decreased suppressive effect may be related to the deficit in the pathway of cell - to - cell contact.
