脑干出血并发中枢性呼吸衰竭患者的撤机护理

摘　　要：目的探讨急性脑干出血并发中枢性呼吸衰竭患者行机械通气后期采用间断撤离呼吸机过程中的临床护理。方法回顾性总结37例急性自发性脑干出血并发呼吸衰竭行机械通气患者在间断撤机过程中出现的护理问题,分析撤机指征和步骤,做好撤机前各项准备工作,撤机时密切观察病情的动态变化,正确撤机,撤机后继续控制感染和治疗原发疾病。结果37例患者在积极控制肺部感染、加强营养、间断撤机等治疗3～7d后成功撤机,机械通气时间为6～35d。结论做好撤机护理工作,配合间断撤机方式是此类患者尽早安全脱离呼吸机的关键。Objective To investigate the clinical nursing methods in the process of mechanical ventilation withdrawal in the patients with acute brain stem hemorrhage combined with respiratory failure after mechanical ventilation. Methods Thirty-seven patients with acute spontaneous brain stem hemorrhage combined with respiratory failure after mechanical ventilation were retrospec- tively analyzed on the nursing problems. The time of mechanical ventilation and the signs and steps of mechanical ventilation withdrawal were investigated. All kinds of work were prepared before mechanical ventilation withdrawal and the dynamic changes were nearly observed. Infection control and the primary diseases treatment were followed after mechanical ventilation withdrawal. Results All the 37 patients succeeded in gradual mechanical ventilation withdrawal after 3 to 7 days＇ treatment of pneumonia control and enhanced nutrition condition. The time of mechanical ventilation varied from 6 to 35 days. Conclusion Intensive nursing of gradual withdrawal of mechanical ventilation played a vital role in the process of mechanical ventilation withdrawal.
